Continuity mistake: Throughout the highway driving sequence in the beginning, Natalie's rear view mirror disappears and reappears repeatedly.

Revealing mistake: Julia confirms that Barnabas is a vampire by sneaking a look at him in her compact mirror - which reflects his human companions, but not him. Unfortunately, the producers seem to have forgotten that vampires don't reflect when, near the end, we see the recently-staked vampire Roger Collins - in a mirror. Not even "dead" vampires ever appeared in mirrors in the TV series, so they shouldn't do so here, either. (01:27:15)

Continuity mistake: Nick's beard is fully grown in the beginning of this episode, but it changes, disappearing entirely more than once and then reappearing in various stages of growth, even in sequences supposedly occurring on the same night.

Mr. Monk and the Red Herring - S3-E10
Continuity mistake: Monk empties a pocketful of pushpins onto the science teacher's counter top. One rolls to the very edge, separate from the others. A few minutes later, the stray pin is missing. Just before Monk and Natalie leave, however, it reappears on the counter edge.

Mr. Monk Takes His Medicine - S3-E9
Continuity mistake: In Stottlemeyer's hospital room, a medicated Monk finishes off the leftover pudding from the food tray. Then, while he's talking to them, the pudding smeared on his upper lip disappears and reappears between shots.

The Her Master's Voice Affair - S3-E1
Continuity mistake: When the taxi leaves the airport, it's a 1965 Ford. When next seen en route, it's a 1958 model with really big tail fins. When it arrives at the house, it's a '65 again, with no tail fins. (00:20:30)

The Foreign Legion Affair - S2-E22
Other mistake: Aboard the plane in the beginning, Illya calls to report in, and forgets his own secret agent number. He tells the communicator, "This is Number 2 Section 1." This is a little like James Bond suddenly calling himself Double-O-Six. Illya worked for Section 2, and was always Number 2 Section 2. Maybe that's just what happens when you're a TV spy and get hit over the head too many times. (00:03:15)
22nd Mar 2006
Visible crew/equipment: When Illya, outside in front of the skyscraper office building, stands beside the cab talking on his communicator, the boom mike is visible over his head throughout the shot. (00:40:00)

The Secret Sceptre Affair - S1-E19
Continuity mistake: Solo, Illya and Zia get out of the car and run into the woods, leaving the rear passenger-side door standing open. When they return, the door has somehow closed itself. (00:36:30)
22nd Mar 2006
Continuity mistake: Aboard the yacht, Solo tucks his gun into his waistband. Next shot, as he takes cash out of the box, the gun has disappeared. It reappears in the shot after that.
22nd Mar 2006
Continuity mistake: In Waverly's office, Illya hones his combat skills by swinging at a suspended wooden block with a baseball bat. When the alarm goes off, Illya puts the bat down on the table, and he and Solo rush to the computer console. In the next, reverse angle shot, the block is still hanging from the ceiling, but the baseball bat has vanished from the table.

Revealing mistake: 1897: Quentin walks away from Trask and exits through the door under the stairs. Unfortunately, that door was undersized, and David Selby was 6'2" tall. Quentin bangs his head on the doorframe hard enough to produce an audible thump.
22nd Mar 2006
Continuity mistake: When the giant cat breaks down the cell door, Korob falls beneath it face down with his head toward the door. In the very next shot, he's in a completely different position - face up with his feet toward the door. (00:43:30)
22nd Mar 2006
Revealing mistake: 1968: A shot of Barnabas and Julia taken through the small window in the cellar door apparently obscured Jonathan Frid's view of the all-important red light. Obviously thinking himself off-camera, Frid took a moment to clean the inside of one nostril with his index finger.
21st Mar 2006
Revealing mistake: 1967: Dr. Woodward struggles with a stuck zipper on an oxygen tent. When the prop refuses to cooperate, he makes a disgusted sound and adlibs to his companion, "Zip that up, will you, Julia?" Then he hurries out, leaving a bewildered Julia to deal with a supposedly critical patient.

Curiouser & Curiouser - S2-E22
Trivia: All the sets in the dream sequence, particularly Nick's loft, are filled with props referencing Lewis Carroll's Alice in Wonderland books. They include white rabbits, flamingos, walruses, a squeaky-toy caterpillar - even a half-eaten piece of cake in Nick's fridge. The episode also has several background extras in Wonderland-style costumes, and numerous other references to the books, such as Nick literally falling through the looking glass at the end.

Plot hole: Spock has absolutely no way to know, yet, that the horta only secretes her corrosive substance when tunneling: he hasn't had time to examine her or to do more than determine that she does indeed secrete a substance that cuts the tunnels. So he should at the very least scan the piece of her that falls off before he picks it up with his bare hands. Major lapse of logic, which given his character goes beyond a character mistake.
20th Mar 2006
Continuity mistake: In the opening scene, the orange-and-white scarf that Paul Harley uses to blindfold Lorissa changes positions and color patterns repeatedly between shots as he escorts her from the pool to the garage.

Mr. Monk and the Panic Room - S3-E2
Deliberate mistake: All the dishes that are usually neatly arranged in Monk's kitchen cupboards (pots, pans, stacks of plates etc.) have been replaced here by cheap plastic ware, so that Darwin the chimp can pull them all out and throw them. Though we hear shattering-glass sound effects as they supposedly hit the floor, the dishes Darwin bangs together and throws are very obviously plastic.

Mr. Monk and the Panic Room - S3-E2
Continuity mistake: Darwin the chimp pulls a large blue bowl from Monk's kitchen cupboard and throws it on the floor, breaking it. We cut to a close-up of Darwin spinning, then pull back as he starts pulling out glasses - and the blue bowl is back on the shelf, intact. When the angry landlord enters, Darwin pulls the same bowl from a different shelf and throws it again.
