Trivia: The box-like entrances to the underground lair proved problematic for the Space Rat actors. When several of them were supposed to spill out on cue, the box walls and their heavy helmets prevented them from hearing the director, who repeatedly screamed "Action!" to no avail. A lone voice from inside the box eventually shouted back, "Can't hear you!"

The Duplicate Man - S2-E13

Revealing mistake: This episode's futuristic video-phones seem to be a production afterthought not in the original script. Consequently, Mrs. James talks to her husband's clone on the phone, but doesn't seem to see him on the screen. She tells James, "He sounded like you," and goes on about his voice, but never mentions his appearance. Later, her reaction to seeing both of them together indicates she's never seen the duplicate before - even though she has, on the video-phone. (00:25:00)

Audio problem: When McGarrett and Danno pull away from the curb in McGarrett's big black Mercury, the tire-squealing sound effect begins before the car actually starts to move. (Doesn't happen unless you're flooring it and "peeling out," which they're not.) (00:41:50)

Trivia: After a homicidal Avon failed to find Vila aboard the shuttle, footage in which Vila emerged in tears was cut from the final version of this episode. (Only a few seconds of it remains.) The emotional scene was deemed "too intense" for the younger viewers.

Revealing mistake: Mr. & Mrs. Castle wish for a million dollars in 5 and 10 dollar bills. But the bills they receive all have the same pattern printed on both sides, a pattern which which doesn't resemble any real US currency. There's also not nearly enough of the bills to total a million dollars. (00:12:30)

Factual error: Outside the warehouse, one of Ballard's men is talking on a car phone. We're not sure who he could connect with, though. The car has no mobile transmission antenna, and in the 60s, it would have needed a very large one. (00:26:00)

Trivia: It's not every day you can shoot your wife in front of millions of witnesses and get away with it. Klyn, the woman behind the console who sounds the alarm before Avon shoots her, was played by Paul Darrow's real-life wife, Janet Lees Price.

Deliberate mistake: Though he moved freely about the station in his environmental suit, Kosh was never actually seen going through a door. He approached doors, but then, sometimes rather disconcertingly, would suddenly be shown on the other side. The reason: look closely and you'll realise his costume was too large to fit through any of the doorways on the set, so in reality the character would never have been able to move around the station.

Deliberate mistake: Because Stephen Greif tore an Achilles tendon playing squash, a stand-in had to complete his scene with Servalan in Ensor's underground complex. The result was a very odd segment in which we hear Travis' voice (pre-recorded by Greif) answering Servalan's lines, but never see him - except for an awkward shot of shuffling, black-booted feet that are all-too-obviously not Greif's.

Trivia: Boorva, one of the planet leaders at the conference, was originally costumed with huge wings on his back. But the attachments kept knocking over props and smacking the other actors in the face every time he moved, so Boorva had to be given a wing-ectomy.

Trivia: A physician who treated Paul Darrow for a chipped ankle bone (incurred in a fall during a fight scene) was appalled to learn he'd be going back on location to do more stunt work the same day, taped ankle or no. "Must you fight the bad guys?" the doctor implored. "Can't you just, well, sit down and talk to them?"
