Plot hole: The mob hitmen are about to enter the Sweeny family's house to kill them when the police van arrives to move them to a witness protection safe house. In the next scene, the bad guys abduct Catherine and try to force her to reveal where the Sweenies have been taken. Why? They could very easily have waited and simply followed the van. (00:36:30)

Continuity mistake: Frank and Catherine drive up to the loading dock and park in front of a bright orange 18-wheeler that's being unloaded in the background. When they get out of the car in the next shot, the orange truck has instantly jumped to a spot much farther down the dock, at least 50 feet away. (00:10:00)

Mr. Monk Is on the Run (2) - S6-E16

Continuity mistake: Monk kicks an oil drum across the floor toward Rawlins. In the master shot, it rolls half the distance and passes a black line on the concrete. The angle changes to a view from behind Monk and Natalie as the barrel keeps rolling and almost reaches Rawlins. But when the master shot returns, the barrel is again just passing the midway line on the floor. (00:25:00)

Continuity mistake: When Murphy questions her on the park bench, Heather's hair changes repeatedly between takes, from tucked behind her ears to having several strands hanging loose in front of her ears. (00:03:20)

Continuity mistake: At the end, when Dale Biederbeck is screaming at Monk, Dale is sitting in his wheelchair with both his hands down in his lap. The shot cuts in the middle of his sentence, and suddenly he has one hand up on the cell bars. (00:40:30)

Factual error: After capturing the gun emplacement, Roosevelt orders his men to "kill the Hun." That particular slang term for German soldiers hadn't yet been coined when the Spanish American War was being fought. It wasn't widely used until many years later, during the First World War.

Mr. Monk Is on the Run (1) - S6-E15

Plot hole: *Spoilers* Monk and Stottlemeyer cook up an ingenious plan to make Monk appear dead, but how the fugitive Monk acquires his regular clothes (in place of Mitch's uniform), the bullet-proof vest and the gun is not addressed. The police are watching Monk's apartment, Disher is watching Natalie and Julie, and Sheriff Rollins is watching Stottlemeyer, leaving us to wonder just how they managed all this right under their fellow cops' noses. Suggested correction: Stottlemeyer is a police captain. The sheriff can't watch his every move, and there are still plenty of cops who are loyal enough to Stottlemeyer for him to get one of them to deliver them to Monk or leave the items somewhere for him to retrieve.

Captain Defenestrator

Nice theory, but there's absolutely no hint of it in the episode. The lack of any explanation creates a bit of a plot hole because it leaves us wondering.

Continuity mistake: The needle Monk finds in the campers' sewing kit threads itself instantaneously between takes. He finds the spool of orange thread, and in the very next shot, tho there's absolutely no indication of any time passing, he's holding the already-threaded needle. (00:22:00)
