The Outer Limits (1963)

The Invisibles - S1-E19

Factual error: The General brandishes a six-shooter pistol with an enormous silencer attached to it. He's a career military guy who should definitely know better, so he gets a failing grade in Basic Weaponry 101. Silencers don't work on revolvers. (00:34:50)

The Invisible Man (2000)

The Camp - S2-E2

Visible crew/equipment: When the van races up to the hospital entrance and the door opens, it reflects a clear image of several film crew members, including the man holding the pole rigging for the boom mike. (00:18:20)

The Outer Limits (1963)

The Invisibles - S1-E19

Revealing mistake: The injured Spain falls and tries to crawl away from the alien, leaving handprints in the dust on the floor. Just to his left, you can see several handprints already on the floor from previous takes. (00:47:30)

Monk (2002)

Mr. Monk Visits a Farm - S5-E14

Continuity mistake: Natalie carries a large box full of house plants into Monk's kitchen and starts placing them one by one on the counter while he answers the phone. Each time the shot changes, a large red flower switches itself from the left side of one plant to the right side and back again, and sometimes disappears altogether. (00:09:00)

Hawaii Five-O (1968)

A Thousand Pardons -- You're Dead! - S2-E1

Factual error: Danno, who's posing as a Marine, has hair that's far too long to be permitted in the Corps. In fact, all of the Marines in this episode have non-regulation (too long) haircuts. 'T'was the same then as it is now: US Marines are given "buzz jobs," and hair longer than an inch is not allowed.

Hawaii Five-O (1968)

A Thousand Pardons -- You're Dead! - S2-E1

Factual error: At the end, Simms shoots at McGarrett and is instantly riddled with bullets from the 8 or 9 cops who shoot back. Yet, when McGarrett walks over to the body, there are no holes - and no blood - marring Simms' pristine uniform. (00:48:00)

The Dresden Files (2007)

What About Bob? - S1-E10

Continuity mistake: At the end, Justin's doppelganger lies down in the coffin and "dies," leaving the lid open. The camera then cuts to a wider shot - and the coffin lid has somehow closed itself. (00:37:00)

The Dresden Files (2007)

Hair of the Dog - S1-E3

Continuity mistake: After Bob's ghost is summoned in the park, Dresden walks off screen right, carrying Bob's skull in both hands. But in the next shot, the skull has vanished. Dresden has neither a bag nor pockets large enough to have put it in. (00:07:00)

The Dresden Files (2007)

Soul Beneficiary - S1-E6

Continuity mistake: Dresden is missing, and Murphy comes to his office for clues. The right side window shade on the front door is pulled all the way down, and still is when Murphy leaves. But when she returns with a rescued Dresden at the end, the right door shade is halfway up, even though no one has been there to raise it. (00:40:30)

Hawaii Five-O (1968)

The Golden Noose - S12-E15

Plot hole: Nadira hides from the bad guy behind a screen - with her feet in plain view. Then, after capturing her, the bad guy inexplicably lets her go, even though her knowledge poses a danger to him. Given that he's killed without a qualm before, this makes no sense at all.

Hawaii Five-O (1968)

Hawaii Five-O (1968)

Season 10 generally

Factual error: 10-13 "The Big Aloha": McGarrett compliments Palahana on his surfing skills with the line, "Nice ride." But the ocean behind them in the shot is virtually flat. With no waves, Palahana's supposed "nice ride" is impossible.

Hawaii Five-O (1968)

Season 10 generally

Factual error: 10-4 "The Friends of Joey Kalima": Guest star John Rubinstein (son of classical pianist Artur Rubinstein) guest stars as Joey - but his name is misspelled in the end credits as "Rubenstein."

The Dresden Files (2007)

Hair of the Dog - S1-E3

Continuity mistake: As Harry talks to Murphy from inside the jail cell, his arms are wrapped around two bars with a third bar in the center. When the shot cuts to a wider angle, his arms are no longer draped through the bars: he's instantly standing straighter and holding onto the center bar with both hands. When it cuts back, he's in his original position again. (00:20:05)

The Outer Limits (1963)

Don't Open Till Doomsday - S1-E17

Continuity mistake: Harvey peeks into the small hole in the box using his right eye. On the first view from inside the box, a right eye is indeed looking in. But when the monster appears, a left eye is peering through the hole. The shot cuts immediately back to Harvey, who still has his right eye pressed to the opening. (00:06:10)

The Outer Limits (1963)

Controlled Experiment - S1-E16

Continuity mistake: When Phobos sets up the time converter, the grid on his large round scope has a diamond-shaped pattern. But when we see it from his point of view, the grid comprises perfect squares instead, and contains numbers that weren't there before. (00:15:00)
