Deliberate mistake: 9-12 "The Bells Toll at Noon": When McGarrett picks up Danny and drives away in his Mercury Grand Marquis, the rear view mirror is missing, probably removed to prevent it from blocking the actors' faces. It reappears later on. (00:33:00)

The Night of the Underground Terror - S3-E19

Factual error: The small round table in West's railcar parlor has a flip-over top, with secret maps hidden on the underside. The wine decanter and glasses all stay attached when it flips. But impossibly, when West turns the tabletop back over at the end, there's a full cup of coffee sitting on it. (00:48:30)

Other mistake: Balya, the Turkish representative, is killed by a knife protruding through the back of his chair. But when he falls to the floor, leaving the knife behind, there's no blood on the blade. (00:42:00)

Revealing mistake: Mr. Braine fires a mini-rocket at Brendage, killing him. But a poor camera angle reveals the explosive charge going off in the wall more than two feet to Brendage's left. From the angle at which it was fired, the rocket couldn't possibly have hit him. (00:21:45)

Revealing mistake: Mr. Braine screams as his steam-powered chair crashes into the bandstand. If you look closely at the next shot, just before the explosion, you can see that the dummy now in the chair has its head bowed forward. It also has light brown hair, rather than Mr. Braine's silver hair. (00:47:10)

Factual error: Though this episode takes place on April 1st, 1969, a freshly-painted sign in the park contradicts the date. In the scene with the hippies in which Sam is disguised as a meditating guru, the huge sign just behind him touts the San Diego Centennial Celebration - in 1968. (00:16:00)

Continuity mistake: West and Gordon are ordered to sit on the floor to talk to the diminutive Dr. Loveless at eye level. In all the two-shots, their shoulders are touching. In all the wider angles, they're sitting a foot apart. (00:09:05)

Continuity mistake: When Gerard leaves the cell block, Kimble is at the cell door with his hands on the bars. Next shot, as Gerard exits, the cell door is visible in the background, and Kimble is no longer standing there. But in an immediate cut back to the cell, he's back with his hands on the bars again. Continuity mistake: Solo and Sheila are against a brick wall with a pipe just behind them as Karmak's leopard approaches. There's a long shot of them edging away to the left, away from the pipe. But in the next close-up, they're back at the same spot with the pipe behind them again. (00:41:55)

Continuity mistake: Sylvia is carrying her huge "opus" sculpture out of the art gallery. In the next cut, though, we see a full shot of her showing both her hands empty: she's not holding the sculpture. Next shot, she has it back in her hands again. (00:24:25)

The My Friend, the Gorilla Affair - S3-E14

Continuity mistake: The giant (and obviously fake) spider is beside Marsha, crawling toward her extended right hand when Illya tells her to freeze. But when he clobbers the foam-rubber menace in the next shot, he attacks the grass behind her. And her right hand, stretched out on the grass when he said "Freeze," has instantaneously moved to her lap between takes. (00:29:05)

Factual error: Ole and all the Thrush baddies call the missing chemical component a "catalyzer," repeating the term throughout the episode. This was a scriptwriter's error, which only David McCallum, to his credit, corrected: he had Illya say "catalyst," which is what the writer meant. "Catalyzer" isn't a word. (00:10:45)
