Continuity mistake: The giant (and obviously fake) spider is beside Marsha, crawling toward her extended right hand when Illya tells her to freeze. But when he clobbers the foam-rubber menace in the next shot, he attacks the grass behind her. And her right hand, stretched out on the grass when he said "Freeze," has instantaneously moved to her lap between takes. (00:29:05)

Factual error: Ole and all the Thrush baddies call the missing chemical component a "catalyzer," repeating the term throughout the episode. This was a scriptwriter's error, which only David McCallum, to his credit, corrected: he had Illya say "catalyst," which is what the writer meant. "Catalyzer" isn't a word. (00:10:45)

The Pop Art Affair - S3-E6

Continuity mistake: The Thrush assassins have an armed golf cart (with a machine gun that handily spews both flames and bullets at the same time). When the cart overturns, the two men fall into a sand trap and tumble several feet away from the vehicle. In the very next shot, they're lying right beside the cart again. (00:02:40)

The Cap and Gown Affair - S3-E30

Continuity mistake: Solo picks up a book, The History of Pacifism, and shows it to Illya, holding it at the bottom. Cut to a close-up insert and his fingers have instantly migrated up to the middle of the book. Cut back to the wider shot, and he's no longer holding the book at all. It's back on the table. Continuity mistake: When Anna tries to kill Koslow, Victor spies on her from behind the ventilation grate. The ribbons fluttering on the duct cover disappear when the shot switches to Victor's point of view. (00:46:50)

Factual error: The jailed student protestors are dancing in their cell to a jazzy tune. But they have no radio or record player - just one guy with a guitar. They seem to have conjured the accompanying saxophone, jazz clarinets and percussion section out of thin air. (00:11:00)

Factual error: Victor is crawling around through Seaview's incredibly roomy ventilation ducts, pursued by several crewmen. But the air conditioning that's fluttering little ribbons on all the vent covers isn't, for some reason, disturbing a hair on any of their heads. (00:44:25)

Deliberate mistake: Throughout series: Five-O was said to be Hawaii's state police force, and was shown headquartered in the Iolani Palace. For decades, tourists were disappointed to learn that in reality, Hawaii had no state police, and that the palace was a state museum, not a state police station.

Audio problem: Throughout series: "Hawaii Five-O's" favorite sound effect was squealing tires, and the effect was nearly always added whether McGarrett's big black Mercury actually "peeled out" or pulled away at a normal speed. More than once, the tire squeal sounded when the car was sitting on beach sand. Nice trick - if you could do it. But alas, you can't "burn rubber" on a beach.

Revealing mistake: Crane overcomes his guard in a fistfight by hitting him in the face three times. But if you watch closely, it's apparent that not one of his blows connects. He "strikes" the air above the man's head all three times. (00:32:00)

Factual error: The Baker City Herald's office door misspells the word "personnel" with a sign reading, "Authorized Personel Only." Pretty poor editing for a newspaper office. (00:29:25)

Deliberate mistake: Several of the roaming dinosaur segments here are repeated 2-3 times over. They're all recycled footage from producer Irwin Allen's own 1960 remake of "The Lost World," and Allen must have wanted to get as much use out of the dino shots as possible. (00:28:00)

The Bomber and Mrs. Moroney - S3-E22

Continuity mistake: The bomber wants revenge on Danno for killing his brother, Joey Collins, whose death is recapped in flashbacks to a first season episode in which Danno first killed someone in the line of duty. But in that episode, Joey's name wasn't Joey or Collins - it was Thad Vaughn. (00:22:00)

Revealing mistake: The one-armed man steals a car and a chase ensues. But during that chase, the stunt driver's two arms and two hands are briefly visible on the steering wheel of the fleeing car. (00:11:00)

Continuity mistake: When Victor tries to get into sickbay, the guard stops him, pointing a rifle at his chest. But in the very next reverse angle shot, the rifle is again in its original position in the guard's hands, pointing at the ceiling. (00:35:10)

Continuity mistake: Barbara searches the one-armed man's wallet. We see an insert shot of two cards, both with the signature "Fred Johnson." But in the next moment, she claims that no two of the IDs are the same before saying, "Oh, wait, here are two." She then "finds" the cards she'd already found in the earlier shot. (00:18:00)
