Continuity mistake: After they get into the hotel and Waikiki is wheeling his brother in a chair, his hand is on the back and his thumb is sticking up in the air. The Nurse gives him a shot and it jump cuts to a close up, and suddenly his thumb is down. (00:09:30)

Continuity mistake: As the monsters are walking down a hallway towards the dance party, Frankenstein has his lobster claw hands and he's walking with them swinging happily out and in front of him with stride. But in the next shot suddenly his arms are down calmly by his side as he walks more casually. (01:07:50)

Continuity mistake: Before diving down to see the underwater volcano, Dracula, Mavis, Dennis, and Johnny are walking on the deck in their scuba gear. They are walking side by side, but in the next shot they are suddenly single file. (00:36:55)

Continuity mistake: Dracula pushes his daughter Mavis out of the attic room when he's on the phone with a witch and shuts the door behind her. Mavis is shown standing there in a shocked expression with her back to the door. But in the next jump cut, she's suddenly turned 90° and has her hand on the door handle, frowning. (00:11:55)

Plot hole: When The Ray is injured on Earth X and has Red Tornado's cortex, he escapes by Vibe sending him into a portal to safety. However it is evident that Vibe doesn't know where he sent The Ray or where he would be going as later he's trying hard, searching the multiverse for him. Yet somehow The Ray is able to go across universes and winds up coming out right at Earth 1's Ray's feet to transfer his powers over to Ray and give him the Cortex. This makes no sense. The Ray can't control Vibe's powers or where he sends him, so he would have had no way of making himself exit the portal in Ray's back yard. And if Vibe sent him there on purpose, he wouldn't have been struggling to figure out where he went. The only way it makes sense at all is for it to be the most massive co-incidence possibly imaginable where he accidentally was sent to another universe and happen to come out right at the feet of that universe' version of himself. None of it adds up. (00:09:50 - 00:16:50)

Plot hole: There was only one T-Rex on the island. When Owen and the others are running down the side of the island escaping the volcanic ash, another dino tries to eat them but then the T-Rex shows up and kills it. This establishes the T-Rex was there in that spot. Owen and the others then wash up back on shore and find the soldiers loading the dinos on a boat and stow away. Then when Blue is bleeding out they have to get a blood transfusion and the only viable candidate is the T-Rex who is now captured and sedated on the ship. How did the soldiers capture, let alone bring the Rex to the ship from where it was seen by Owen back up over hills and rocky terrain? It would have been an all day process just to haul the T-Rex to the ship. It just magically appears on the ship after they stow away on it and can provide the blood Blue needs.

Bug: Multiplayer Burial Mounds: If you smash another player against a certain pillar with a vehicle, they will fall through the map. If they are lined up right, they fall onto an orb out of map that is actually the sky box housing for the textures around the map. They will partly fall into it and their image will then suddenly be plastered all over the sky as a gigantic Master Chief corps.

Trivia: A long running mystery of this game, there is one spot where an abandoned building is off in the distance. If you unlock the camera and head to that spot, you can find an odd figure of a man standing there. It is unknown who this is or why it's there, being an unfinished abandoned area of the game, or a detail, or just some odd bug. One theory is that it seems to resemble one of the game developers Shigenori Nishikawa.

Character mistake: After Wade is going over the second clue in his mind and trying to figure out what part of Halliday's past is he trying to escape from, he looks over the wall of article clippings and notes he has hung up. When the camera stops on Karen Underwood's obituary, there is a hand written note beside it listing movies set in New York, with the sentence "Set in New York" underlined. This apparently him looking for clues in films set there for the race challenge that is in Manhattan. However, one of the films he has written on that list is Mad Max, which takes place in Australia, not New York. (00:35:00)

Other mistake: After Wade gets the second clue, he's shown in his little hide away spot looking over a notebook he has wrote in with the words Clue #2, a large number 2 out next to that, and under it in quotes "A creator who hates his own creation" with a hand drawn box around creator. This sentence is highlighted and there is more under it. This is a hand written note on a spiral notebook that he just got after getting the second clue which had only now been revealed. So he sets the notebook in his lap and starts looking over the newspaper clippings and other papers on the wall about Halliday. As the camera pans over, you suddenly see the top of the same paper torn and pinned to the wall. The hand writing is exactly the same, the boxes and figures drawn exactly the same, and is only missing the orange highlights over it. The paper is wrinkled a bit and shows the holes from the spiral as it was torn out and pinned to the wall. But this is impossible as he still has the same paper in his hand, and he only just now got this clue. Somehow a duplicate is on the wall already. (00:34:20)

Trivia: Daito's personal vehicle in the OASIS is the Mach 5 from Speed Racer. You can see it in the background as he is receiving the Copper Key, and it is also used in the final battle. (00:33:45)

Trivia: If you use a glitch in a certain spot, you can lift yourself up out of the map. This will lead you out of bounds to a highway segment of the map that is usually inaccessible to players. However, if you get hit by one of the cars, it triggers a death animation for your character that is unused anywhere else. This indicates that this area was meant to be playable at one point, but was cut out later in development.
