Emergency! (1972)

Back-Up - S4-E21

Continuity mistake: When the cardiac patient is finally brought to the ER, and goes into full arrest number four, he's wheeled into treatment 1 which does not have a window on its door, but in the next shot from inside the room the door has a window, and when they're defibbing him, we can see the nurses' station outside the door, which means he's now in treatment 3, not 1.

Back-Up - S4-E21

Continuity mistake: After leaving the football player who kicked in his TV screen, as Roy and John put the gear back in the squad, we see the old blue set of the Datascope 850 monitor and defibrillator, and not their current Datascope M/D2 white case. The last time the blue two-piece unit is even seen in the squad is in 4x13, "Parade", and the first time the white unit is used is in 4x15, "Transition", and it's then used in all following episodes. This scene was a prior taping, that was inserted into this end of the season episode.

The Old Engine Cram - S5-E2

Trivia: After John and Roy dial the wrong number twice, it cuts to a shot of LACoFD headquarters, and the dispatcher, Sam Lanier, transmitting the paging tones. In the closeup of the Motorola Quik-Call, note the company numbers on the tabs and that all FD companies are in numerical order except for 51, which is placed beside 127. (Can also be seen in other episodes).

Election - S5-E3

Revealing mistake: When Station 51 is dispatched to 'man trapped', just as Engine 51 rolls out of the apparatus bay and heads down the driveway, the real 127's vehicle is seen parked inside the bay, near the rear door.

Election - S5-E3

Continuity mistake: After bringing the man who choked on a beer can's pull tab to Rampart, when Squad 51 returns to the station and backs up the driveway, the engine seen parked in the apparatus bay is the Crown from seasons 1 and 2, not their current Ward LaFrance engine.

Kidding - S4-E17

Revealing mistake: After Station 51 is dispatched to the 'plane in trouble' it becomes a plane crash rescue and recovery, and when they pull out the pilot's body, who they said died on impact, his right hand moves to clasp his left wrist, and he also bends and moves his leg.

Kidding - S4-E17

Other mistake: When Dixie and Mr. Hart are on the elevator, we see the number five lit up on the panel above the door as they head down to the first floor in the ER, where they meet Johnny and the kids, who get on the elevator to head up to the fourth floor. The problem is that the ER is only a 2-storey building.

Kidding - S4-E17

Continuity mistake: When Brackett asks Early and Morton to assist with the examination of the hallucinating veteran, in the shot from the hallway the doctors walk into treatment 1, its door has no window, but when it cuts to the shot from inside the room, its door has a window, which means they're all in treatment 2 or 3, not 1.

Smoke Eater - S4-E16

Continuity mistake: When Billy's mother waits in the hallway while Early examines Billy, the window behind her is on the same side as the ER's receiving entrance. Note that outside the window there's a blue metal shade at the top, which does not exist in any exterior shots of that side of the ER.
