Mind Games - S3-E3
Revealing mistake: When O'Neill starts taking Polaroid photos of Cassidy in the barn, Cassidy's blouse is properly buttoned up, but the second photo O'Neill takes shows Cassidy's blouse is being held together with a large safety pin, then the buttons are intact again. (00:39:10)

Visible crew/equipment: After Tony gives McGee the coffee belonging to Gibbs, when it cuts to inside Laura Rowans' house, just as the Sergeant tells Gibbs, "The perp broke in, tried to rape her, she shot him," the boom operator (wearing a blue shirt) holding the boom pole is reflected on the framed picture's glass. He's visible again when Caitlin and McGee walk in. (00:02:30)

Visible crew/equipment: After Gibbs tells Tony to go down to Forensics to go over the physical evidence, when it cuts to Ducky walking into Autopsy while saying good morning to Jimmy, the reflections of the camera operator and camera are visible on the steel door. (00:34:30)

Visible crew/equipment: While in the morgue, Ducky asks Palmer if his recorder is on, and in the next shot a crew member wearing an orange shirt is visible on the right side of the screen. Visible crew/equipment: After Gibbs meets with the real Colonel Walsh, when he's back at NCIS Gibbs asks for Morrow's help to gain access to records, and in the overhead shot a crew member's leg (wearing jeans and a sneaker) is visible on the left side of the screen, directly behind Morrow. Visible crew/equipment: When Nasser is brought to the interrogation room he raises his handcuffed arms and Gibbs nods no to removing them. In the next closeup of Nasser's handcuffs we can see the black taped mat on the floor (also visible in a following shot) which doesn't exist in overhead shots, then as the camera tilts up the reflection of the camera operator (wearing shorts) and camera are visible in the mirror. Visible crew/equipment: While Caitlin and Jane Doe are talking about blueberries it cuts to the doctor telling Gibbs that he can't release Jane Doe, and at the start of the next shot the pink T-mark is visible on the floor in front of Gibbs. (00:13:10)

Continuity mistake: When Gibbs rejects the agent's file, Cynthia reaches for the ringing telephone with her right hand and Gibbs stops her with his left hand, but in the closeup Gibbs has his right hand on Cynthia's left hand. The closeup is a flipped shot, note their watches. (00:03:05)
