Continuity mistake: In the elderly victim's home, when Gideon is going through the dresser drawers he finds the small frame with the picture of her son, and there is a blue frame lying in the drawer, but in the next shot that blue frame is lying on the dresser even though he never touched it. Visible crew/equipment: After Miss Kizlet enters her office to find the walking base station Doctor sitting at her desk, as she walks around the motorbike lying on the broken glass, when this 'Doctor' says, "Yes, those still with bodies to go home to would be free," we can see the moving reflection of two crew members on the surface of the motorbike.

Continuity mistake: When the Doctor and Clara are on the rooftop terrace, there's a distinctive design on the Doctor's bow tie, but when he's in the coffee shop the bow tie's design differs, and when he runs outside to ask if Clara's okay it changes back, then it changes again when he's in back the shop. Additionally, when the walking base station Doctor uploads Clara it's wearing a bow tie, and its design also changes on the motorbike and in Kizlet's office.

Continuity mistake: While Clara shouts, "I don't know where I am," over and over, the Doctor sonics his way into the house. When he kneels beside Clara, then notices the 'girl' on the stairs and stands up, in the shots facing him we can see that the young actress stands on the step facing the stairway (note her legs), with her hand at her side, but in the shots facing the girl she stands facing the door with her hand on the banister. Continuity mistake: Miss Kizlet wears a large silver brooch on her suit's right lapel in every shot of her, but after Clara notices the lights going out behind her house, when it cuts back to the office Kizlet's brooch switches to her left lapel just before it cuts back to the Doctor and Clara, and since the brooch has a distinctive design we can tell that it's a flipped shot. Revealing mistake: While everyone is pinned down, when Jothee shows up in the stolen Scarran ship and fires on the enemy, in the overhead shot we can see the special effects canisters that cause the explosions on the ground.

Visible crew/equipment: During the escape, Aeryn comments that there are too many fresh boot prints and they're not alone, then a few shots later after D'Argo shouts, "Get back! Get back!," in the next shot the springboard can be seen propelling a Peacekeeper stuntman.

Visible crew/equipment: During the escape with the Eidelons, when Aeryn's holding the baby and says, "There's too many boot prints and they're fresh. We're not alone," a moment later there's an explosion, then after she shouts, "Inside," and leaves, in the next wide shot the springboard can be seen propelling an Eidelon stuntman. Plus the fact that in this shot Aeryn is still by the wall.

Visible crew/equipment: After Aeryn delivers the baby and asks John if he can get them out of there, in the shot after they kiss there's an explosion, and as the Charrid stuntman is thrown into the air we can see the springboard's top platform flip up, which propelled him into the air.

Revealing mistake: When John uses Scorpius as a shield and takes cover in front of him, as John fires at the Tregans behind Scorpius, there are round black devices attached to the back of Scorpius' suit that are there for the gunfire special effects, which suddenly appear then disappear after.

Visible crew/equipment: While Moya is under attack by the Tregans, after D'Argo asks Pilot if Moya can free herself from the hauling lines, when Pilot shouts, "The Tregan ship is pulling us in," we can see the puppeteer rods under Pilot's arms and also the fingers of one of Pilot's puppeteers.

Continuity mistake: When D'Argo, who is followed by a few Eidelons, is walking to meet Moya's pod (with Chiana aboard), there are short shadows directly beneath D'Argo in shots facing him, but in the shot facing the pod there's a long shadow being cast in front of him.

Visible crew/equipment: After D'Argo finds the Peacekeeper control panel and rips out the wire, he's thrown back and slides down a shaft. When he stops sliding we can see the safety cable behind him just before he finds the Peacekeeper shield, and then when he continues to slide down the shaft we can see the safety cable once again.

Visible crew/equipment: After D'Argo and Zhaan agree to be allies against Rygel, it cuts to Pilot's den and when Pilot tells Aeryn that he's in "nominal" pain, two puppeteer rods can be seen under Pilot's arm.

Continuity mistake: When D'Argo, Aeryn and John start their search for clorium, D'Argo is wearing arm bracers which extend over the palm and back of his hands. After John says, "Pilot said you press this, this and...this. Works just like a VCR, except easier," D'Argo grabs the particle analyzer from John, but in the closeup of the particle analyzer the hands we see are bare, which means it's not D'Argo, then in the next shot D'Argo is still holding the device.

Visible crew/equipment: After they all take turns peering into the triangular hatchway, Pilot silences the paddac beacon's siren, and when the DRD zooms into the compartment in the shot from within the room we can see a cable behind the DRD.

Visible crew/equipment: After Aeryn extracts the worm from John's abdomen, when Rygel says, "If I were warmer, I would have an appropriately venomous reply. Be warned, I owe you one," he drops down off the table, and as Rygel waddles away the boom mic dips into view at the top right side of the screen.

Revealing mistake: After Rygel realizes that he can loosen the soil with the dumped gruel, it cuts to John and Aeryn as they approach the Tavlek camp, and when Aeryn tells John, "I am going to take out every last Tavlek. No survivors. No mercy. Now give me the oculars," John holds up the syringe (with Zhaan's sleep mist) that Aeryn had given him earlier, and we can see "DG-4" engraved on the side of the gun, revealing that the prop dept used the model DG-4 aluminum air blow dust gun to play the part of the syringe.

What Was Lost (1): Sacrifice - S4-E2
Continuity mistake: After Oo-Nii tells John that the old woman stole his gun to kill Vella, John looks down at his holster and finds Winona gone. Later while he's shouting for Granny, it keeps cutting to the old woman while she's holding Winona, and after she whispers, "Receive the spirit," it cuts to John and Winona can be seen in the holster on John's leg, but in the next shot Granny still has it.
