Continuity mistake: When XXX stuns with rather unorthodox techniques the two guys the idiot co-protagonist left alone to guard the base, the first one to go down collapses with his right arm straight in front and the left hand below his elbow. When Scott Speedman gets back and start chasing Ice Cube, you can see the same man on the floor, right arm all the way down along his side, and his body turned so the left hand is not in the same position. Continuity mistake: In a problem already spotted in the previous movie of the saga, the departed Michael Roof had poor ring discipline; when the scene of the assault at the barn begins and he reminds 'X' that it's a covert operation, he's holding the binoculars with ring-free hands, but in the shot when he sighs saying "Subtle..." he sports an obvious metal band (exactly like in the original movie) at his right ring finger. Continuity mistake: When Gibbons asks Agent Toby Lee Shavers to fit the rocket he picked from the car into a personal weapon, Michael Roof holds the rocket in two different ways (grip on the pipe or between the fins) depending on the camera angle. (00:20:10)

Continuity mistake: When Ice Cube reverses the Gibbons' "Three strikes" speech, the waitress behind him keeps changing position throughout the scene, serving the table to the right of the door, or to the left, back and forth. (00:14:40)

Continuity mistake: When Ice Cube fights three guards on the rooftop, the light varies significantly; just look at the guards when they are on the ground, the first one in particular, and where the shadow of the building extends. (00:12:40)

Continuity mistake: After for some incredibly silly reason the sniper does not shoot Darius thus ending the movie early, the convict jumps off the roof and finds more guards to beat up. He faces the first one, punches him, a new one joins...only, not, since the next shot is of Ice Cube kicking some more the first, while the second is just now coming out of the door giving him more time to kick more butt. (00:12:20)

Continuity mistake: Darius Stone is fighting the first two wardens, the ones who have him in cuffs. One of them decks the other by mistake with the baton; the stuntman goes down on the ground next to Darius, face first, in the wide angle, but the actor is limp with his back to the door in the next shot. (00:11:00)

Continuity mistake: The President's assistant asks him the first time we see him "Roses or tulips?" George appears out of the blue in the next shot despite being barely (perhaps) visible at the very end through the side door, while the other people in front of the President were exactly where they are in the new shot. (00:06:30)

Continuity mistake: When Shredder unbalances Donatello in the fight his left foot is close but not inside the large shadow area, but Michelangelo attacks him right away and despite his lack of movement, now that foot is in the shadow cone. (01:20:20)

Continuity mistake: The Turtles are making fun of The Shredder's name once he announced it. Look at Michelangelo when he recites the coleslaw joke; his bandanna goes down the right side of his back, but it's on the left side in the next shot. (01:19:05)

Continuity mistake: When the Turtles strike a pose after they wasted all the Foot soldiers before their big showdown with The Shredder, the ninja lie unconscious at their feet in different position. Even the roof is lit differently. (01:18:20)

Continuity mistake: Master Tatsu is raging against the students, and slams one against the lockers. Another Foot soldier tries to restrain him. The unmasked victim of the assault is clutching the shoulder bit of Tatsu's hood, but his hand is further down his arm in the reverse shot. (00:49:00)

Continuity mistake: In the rumble inside the antique store, Mike at one point squeals that they could use Raph, "right about now." Cut to Donatello fighting some bad guys to the left of an unimpressed and passive Master Tatsu. The following shot has Don fighting Foot clan members to Tatsu's right, and it's Leonardo to be battling to Tatsu's left (right side of the screen). (00:45:30)

Continuity mistake: Mike thanks to Don's help does the "Wheel of Fortune, dude!" move; the floor next to him is relatively clear from debris and he sweeps it, but when the bad guy throws a single axe strike next to his head, the floor is all messed up and cut. (00:44:00)

Continuity mistake: In the rooftop fight, as Raphael quips about the Foot guys studying the abridged version of the Ninja book of fighting, you can see his belt is hanging kinda low, at the hips almost. In the next shot, Raph is fighting with his belt in senior citizen mode, pulled all the way up to 2/3 of the torso. You can tell quite easily because being a turtle his belly is divided in sections. Continuity mistake: April is laughing at Mike doing a Rocky impression. Camera behind her shoulder; notice her hand slips off the table. Back to a frontal view, and her hand rests on the table as it was before. (00:26:10)

Continuity mistake: When April tries to rationalize what she's seeing, says pointing at the Turtles; "And you guys.. " Mike and Leo are looking at her. Leo's arms are low, but in the reverse shot they are held higher towards her. (00:22:25)

Continuity mistake: When Xander switches off the submarine by tearing off its HD, the flame of the rocket wildly varies in intensity and colors in each shot of the sequence. (01:51:00)

Continuity mistake: Harpoon in hand, Xander yells to his driver "There's a bridge" pointing at a railway bridge with characteristic metal archways. "Cross the bridge!" he orders, and they do cross a bridge that is blatantly different, with no metal structure. (01:46:20)

Continuity mistake: Asia Argento unties Xander with a knife she had strapped to her boots. In close-up, the boots are army-like with a dark olive color, but when she was sitting down, and even better when she later has to put her feet on the car pedals, she is sporting pointy camel-yellow boots. (01:35:30 - 01:45:40)
