There's Something About Paulie - S2-E16
Other mistake: After the car bomb destroys their new blue car, the groom at the wedding comes out to warn them too late, then runs off. As Peter and Lois are turning around in the next shot to walk, the cars behind them are disproportionally small, with the scaling being off here. (00:21:35)
Continuity mistake: When the kids are playing Duck Duck Goose, after it's Stewie's turn to be it, there's suddenly fewer kids in the circle. It's possible the darker-skinned boy is off screen on the right side; however, the girl on the left has fully vanished, with the kid in the orange shirt taking her place. (00:05:40 - 00:06:10)
There's Something About Paulie - S2-E16
Other mistake: Peter follows the mobster to the "pet store" and is standing on the sidewalk when the mobster gets out of his car and goes inside. The street here is comically disproportionately small. So much so that when Peter walks across it, he clears the entire road in two steps. (00:14:40)
There's Something About Paulie - S2-E16
Continuity mistake: When Big Fat Pauly is eating dinner with the family, the thermostat on the wall is solid white. A few scenes later, when Lois and Peter are talking at the table after Pauly's death, the thermostat is now grey with a blue and red dial. (00:09:55 - 00:14:10)
There's Something About Paulie - S2-E16
Continuity mistake: When Peter tells Lois he accidentally put a hit out on her after she sees Big Fat Tony dead in the newspaper, the paper is set on the table in front of Peter by a yellow box. During this conversation, the paper and the box keep disappearing or moving positions between shots over and over. Also, Peter's cup disappears in a couple of shots. (00:14:00)
There's Something About Paulie - S2-E16
Continuity mistake: As Peter is showing Cleveland his new car, it is in the middle of his driveway. Then, when Peter is getting back out of his car, suddenly it's in the street. (00:06:00)
There's Something About Paulie - S2-E16
Continuity mistake: Once they have a new car, Lois is telling Peter to stop winking about the car. They are outside next to the house, and their screen door is behind them. Then, in the next shot, suddenly they’re at the corner of their house next to the garage door. (00:05:20)
There's Something About Paulie - S2-E16
Continuity mistake: After Peter and Lois' fight, the next day Peter is in a deli where apparently Cleveland works. Cleveland sees Peter is upset and asks him if he "wanna talk about it, champ?" He is frowning in that shot; in the next shot, he's suddenly smiling. (00:03:55)
There's Something About Paulie - S2-E16
Deliberate mistake: At the dealership, Lois pops the hood of the red car and shows there is no engine in the vehicle. She pulls out a paper that just has a drawing of an engine. Yet moments later, Peter is able to start the car and rev a nonexistent engine and drive it a couple of feet before the wheel falls off. (00:02:40)
There's Something About Paulie - S2-E16
Revealing mistake: At the dealership, when Peter is "flirting" with the first red car, he says, "What's that? You want me inside you?" Just as he's saying "What's that," his right arm clips through the windshield of the car and door, phasing through the car like air. (00:02:15)
There's Something About Paulie - S2-E16
Continuity mistake: At the car dealer, Peter walks up to a car and says it's a bad little car and does it want Peter inside it. As he's climbing in, Lois suddenly appears, popping in on the right side of the screen. She does not step into frame; she just suddenly pops in. (00:02:15)
There's Something About Paulie - S2-E16
Continuity mistake: During the taxi cutaway, the guy talking on the mic is in a booth with papers on the walls. The shot cuts to the guy in the white shirt at the table next to it, and suddenly the booth walls and papers are much larger. Also, the paper directly behind this guy goes from only partly covering the wall to nearly covering it fully and being end to end. (00:01:30)
There's Something About Paulie - S2-E16
Continuity mistake: When Meg is explaining why they can't do Death of a Salesman in school, Lois is lying on the couch with her feet visible. Peter is not shown in this shot, but just before, he was standing behind the couch. Then, in the next shot, suddenly Lois is sitting upright and Peter is sitting on the couch right next to her. (00:01:10)
There's Something About Paulie - S2-E16
Other mistake: Peter walks into the living room and finds Lois on the couch. Just after he asks her if she's been drinking, she starts to sit up and then suddenly the entire episode starts bobbing up and down. Like the show is on a camera bouncing in a car. This lasts a couple of seconds then stops. I checked on Disney+, Hulu, and WCO. Each of these had the same camera jitters in that moment. (00:00:45)
There's Something About Paulie - S2-E16
Continuity mistake: Right at the start of the episode, Stewie is walking up to Lois carrying a lamp. The camera cuts to a closer angle, and then the lamp is now a lot thicker or fatter in size. (00:00:30)
Continuity mistake: When Stewie walks up to Janet during nap time, the kid that was asleep behind Janet has suddenly vanished, leaving an empty mat for Stewie to lie on. (00:20:15)
Other mistake: After the Kentucky cut away with Peter, we're back on the plane. He sits down and is surrounded by the exact same other characters from the last plane we saw him on: the pink lady sitting next to him and the man in the green suit sitting behind him. This is after at least a day or two in the episode, and Peter is flying all over the world on different planes as a freeloader with his wife's new job. It's next to impossible that he'd be sitting next to the same lady and in front of the same man again. (00:09:30 - 00:15:10)
Continuity mistake: In the Kurt Cobain cutaway, it shows him and his band on stage. When it cuts to a close-up of him, suddenly his drummer and the other guitarist vanish. (00:11:55)
Continuity mistake: When Lois is telling Peter she's thinking of quitting her new job as a stewardess, she leaves her jacket on the corner of the bed. After Peter convinces her not to quit yet, her jacket has vanished off the bed. (00:11:50 - 00:12:20)
Continuity mistake: The pilots prank Lois on the plane at her new job as a stewardess by pretending to be stabbed to death. While they are laughing at her, she straightens up and the camera cuts to a close-up with her saying, "Bastards." In this shot, the frame around the cabin door disappears. (00:09:00)

Answer: Lacey Chabert was the actress who originally voiced Meg in early episodes. She left of her own accord due to being in school and other acting work; Seth MacFarlane has stated there was no tension with her leaving, but it makes for a handy joke for the show.
