Continuity mistake: After the paint factory fire, when Roy and John are with the lawyer in Captain Stanley's office, Roy's chair is either directly in front of the desk or off to the side, depending On Camera angles. Their body positions change between shots as well.
Other mistake: During the fire at the paint factory, just after the barrels at the back of the pickup up truck explode, in the next shot we see Marco and Chet manning a hose. In this closeup, another fireman runs up to them and he's wearing a helmet with 51 on it, but it's obvious he's not one of 51's guys, plus Roy and John are with the burn victim and Mike's at the engine. The fact that it's a closeup and we can see perfectly well that he's not actually from 51 makes this too odd.
Other mistake: After Mr. Winthrop is brought to the ER, there is a closeup of his EKG monitor as it's picking up multiple PVCs, but when Brackett asks him about the tension in his life, the monitor seen behind him is showing a flat line. This happens once again after Roy and John leave the treatment room, just before they move him to CCU.
Other mistake: After Roy and Johnny get back to the station, when Johnny takes the container of milk out of the fridge, we can see the black tape around it covering the brand name. There's another container in the fridge with red tape covering its brand name as well.
Visible crew/equipment: When Station 51 is dispatched to The Boat explosion, just after Captain Stanley responds, "KMG365," as he walks past the camera to the engine, we can see the open top of the set at the rear corner of the apparatus bay.
Revealing mistake: During the fire at the apartment complex, there are strips of black material nailed to the tops of windows where the FX fire shoots out from below, and the doorway Roy passes when he's pulling on the hose has a fake ceiling and wall nailed directly into the door jamb, to control the strip of FX fire at the side.
Revealing mistake: At the apartment complex fire, just as Chet climbs the ladder and Marco holds the top of the ladder on the second floor balcony, we see the plumes of FX smoke spurting from the side inside the doorway behind them, and even see something quickly fanning the smoke.
Continuity mistake: When the boy who fell over the cliff is brought into the ER, Dix tells them to bring him to treatment 3 and we see them at treatment 3's door, which is on the same side of the hallway as treatment 4, with the red line closest to it (hall has red, green and yellow lines on the floor which run the length of the hall) However, in the shot from inside the treatment room, they're not in treatment 3, but are actually in treatment 1. Note that we see treatment 4 directly across the hall, which means they're in treatment 1, and also see the yellow line is closest to the door which means it's on the opposite side of the hallway.
Other mistake: When Roy's talking to Dixie while John's waiting out in the squad, a call comes over the HT that Roy's holding, and Squad 51 is asked to respond to a 'possible heart case'. Roy pulls a piece of paper out of his pocket, and as we hear the dispatcher giving the address, we can see Keven Tighe softly mouthing the address at the same time that it's being given for the first time.
Continuity mistake: When Johnny, Roy, and Chet are at the site of the traffic accident, during this scene the shadows' lengths - from long, to short, back to long, and its directions keep changing drastically. Such as when Chet stands in the middle of the road, looking in the direction of the vehicle with the siren, his long shadow is cast to his left, but the oncoming vehicle also casts a shadow to its left (even though it's heading towards Chet), then in the next shot that vehicle's shadow has moved to its opposite side.
Continuity mistake: When Brackett is in his office talking to Patsy's manager, he's paged and then told, "Treatment room 2, STAT," and he answers, "On my way," but Brackett doesn't rush to treatment room 2, he goes to room 230 which is Patsy's room, and Early even calls for a code blue in room 230.
Continuity mistake: When Brackett and Morton are with Patsy in her hospital room, there are two closeups of the Datascope 850, but there's an entirely different unit at the foot of Patsy's bed monitoring her EKG, not the Datascope 850. The same thing happens again later, when Brackett rushes in and Early tells him that Patsy was throwing more PVCs.
Continuity mistake: When the guy who was injured in the manhole is brought into the ER, Dix tells them to bring him to treatment 6 (which is around the corner from the other treatment rooms, and has no color lines that run past its door), then we see him wheeled into treatment 6 in the shot from the hallway. However, in the next shot from inside the treatment room we can see they're not in treatment 6, but actually in treatment 1, note the red, green, and yellow lines on the hallway floor in front of its doorway, and treatment 6 has no color lines that run past its doorway.
Visible crew/equipment: While en route to the structure fire, Mike is driving Engine 51, and seated backwards in the jumpseat behind Mike is Marco, who's wearing a helmet and his turnout, but in the shot from the POV behind Mike, we see the reflection in the rear-view mirror of the grey haired crew member, who is seated facing forward behind Mike.
Visible crew/equipment: During the structure fire, when Captain Stanley uses the HT and says "Engine 17, you can pump to Engine 51," in the next shot a second cameraman carrying a handheld camera is visible between Engine 17 and Engine 9, as he's shooting footage. He's visible again three shots later, standing beside Engine 17, and once again right after they lower the ladder through the hole in the roof.
Other mistake: At the structure fire, Roy and Johnny rescue 17's Captain Harmell, as seen on his helmet after he fell through the roof, but when Truck 17 sends its snorkel to the roof, 51's Captain Stanley and Captain Watson, whose helmet also shows 17, both assist with the rescue of the other Captain of 17.
Audio problem: When Roy and Johnny are tending to the actor with possible MI, Brackett asks for an EKG strip, and after Johnny hooks the scope up to the Biophone, we hear him say, "Lead 2," but his mouth is closed and doesn't move.
Other mistake: While the guys are in the kitchen, deciding whether or not to keep the dog and then what to name him, when Chet is drinking the milk, we can see the red tape covering one side of the container to hide the brand name and logo.
Other mistake: When Squad 51 is dispatched to the movie studio, while en route in the interior shot, in the rear-view mirror we see the reflection of the driver's helmet, and it has an 18 on it, instead of 51.
Other mistake: When the boy is brought to the ER by his babysitter, they hook him up to the ventilator and we see the bag dramatically showing inflation and deflation in the closeup, but in the wide shots the bag is deflated and doesn't move.
