Character mistake: When the guy who was injured in the manhole is brought to treatment 6, he's transferred onto the ER's gurney and we see Carol taking his pulse, but then she tells Early, "Pulse 120, respiration 24," even though she never checked his respiration rate.
Continuity mistake: When Deke comes into the ER with the two burned firemen and speaks to John and Roy, the window behind them is on the same side as the ER's receiving entrance. Notice that outside the window there's one of those blue metal shades at the top, which does not exist in the exterior shots of that side of the ER.
Visible crew/equipment: When Roy, John, and Vic are inside stage 3 trying to outrun Charlene, Roy starts climbing down the first ladder, and we can see equipment moving right beside Roy, then when Roy climbs back up and they find the second ladder, more movement can be seen at the bottom of the screen.

When Roy and John are there the first time, it's for no more than five minutes. Then when they're there the second time, Roy says "Please folks, we just went through this less than an hour ago," though it was over two hours when they left the fighting couple the first time.

Continuity mistake: When Roy and Johnny get back to the station, after the cave-in rescue, the dog is lying on the couch on the middle cushion, between Mike and Marco, but in Henry's closeup Mike is gone, and the right side of the couch is empty, then Mike's back, but he vanishes and reappears again.

Continuity mistake: At the singles club fire, after Roy and Johnny go in to look for the missing man, we see Mike charging the lines, then there's a shot of two of 16's guys walking past the stairway with a hose, and notice that on the stairway we can see Johnny's and Roy's legs heading down the stairs. This same shot is shown again after Roy and Johnny check the men's room upstairs, and once again we see Roy and John coming down the stairs when 16's guys walk past them.
Continuity mistake: When Squad 51 is dispatched to the possible heart attack, the address given is 11044 Conquistador, but when they arrive at the incident the addresses on the homes are 8042 and 8046.
