It's How You Play the Game - S4-E19
Continuity mistake: At the scene of the vehicle accident, when Roy's on the Biophone with Rampart the squad's lightbar is on properly, the center chrome piece has vertical slats that face forward. However, after the victim is removed from his vehicle, when Johnny's on the line with Rampart note the squad's lightbar again, the center chrome is now on backwards, the solid side faces forward.
Camera Bug - S4-E10
Visible crew/equipment: After Johnny snaps the picture of Marco on the engine, when he prods Chet to go stand by the map, as the camera moves to the left we can see the reflection of production equipment/crew on the back of the squad, and we can even make out the shape of someone seated on a director's chair.
Camera Bug - S4-E10
Visible crew/equipment: When the stretcher with the unconscious teenager from the school fire is wheeled into the ER, while Roy is using the Ambu bag, just before they reach treatment 3 we can see the top of the set down the hall, near the nurses' station.
Continuity mistake: Squad 51's lightbar has a center chrome piece which has slats at the front and is solid at the back. When the squad's dispatched to the vehicle accident with the two student drivers, on scene the lightbar's chrome center is on backwards, it's solid at the front for some inexplicable reason.
Continuity mistake: When Chet is brought into the ER after being thrown in the explosion, Johnny is empty handed, but when he leaves treatment 1 with Dix, the HT is suddenly inexplicably in his hands.
Continuity mistake: During the actual rescue of the boy from the overturned car in the water, Roy's, Johnny's, and Chet's shoes are water-type shoes, which then change back to their uniform shoes.
The Lighter-Than-Air Man - S5-E8
Visible crew/equipment: When Johnny brings in the box and Chet removes the wrapping for the baby layette, note the unfinished set wall past Engine 51 in the apparatus bay, which we can see since the front bay door is open, but when Captain Stanley walks in telling John about the phone call, the front bay door is now closed, then when the tones drop that set wall can be seen once again.
Continuity mistake: While Roy and Johnny are studying, after hearing Roy's positive horoscope Johnny tells Chet his birthday is August 28th. However, in the pilot episode "The Wedsworth-Townsend Act," Dixie has a get-together as a combination celebration of John's birthday and gathering of the supporters of the paramedic program. At the party Dixie offers the hors d'oeuvres to Johnny saying, "You first, it's your birthday," as he takes a mouthful of food. During that week we see people wearing coats, and the many deciduous trees (not the evergreens and palms) in LA County have already lost their leaves, which means it's certainly not summertime, August 28th.
Continuity mistake: Before the fake eye test begins Captain Stanley hands each guy their own cheat sheet, and when Chet walks out of the day room upset about the eye test, John puts his cheat sheet into his shirt pocket just as they're being dispatched to the "accident and car fire." After the rescue, when John and Roy get back to the station John joins Chet by the lockers, and when John tries to prove that the eye test was a prank, John opens his locker and pulls out his cheat sheet, even though we saw him place it in his shirt pocket before the run.
Daisy's Pick Blind Date - S4-E7
Visible crew/equipment: After Brackett's examination of Jerry, the unconscious 3 year old, when he goes to the waiting room and introduces himself to Jerry's parents, the top of set can be seen over treatment 1.
Continuity mistake: When 51's guys go down into the tank to try and subdue Al, the big guy who has a chemically induced psychosis, the ring Al was wearing on his left hand in the overhead shots is gone, but when Al shakes Chet upside-down in the closeups the large ring is back, but then it vanishes again.
Other mistake: When the squad and engine roll up on the scene of the bulldozer accident in the junkyard, just as the guy runs up to inform them about the victims, have a look at Squad 51's license plate number because it is not the normal prop plate number E999007.
Visible crew/equipment: When the boy who ingested ant poison is being wheeled into treatment 3, in the shot from the hallway we can see the electrical cord and plug lying across the floor, but in the next shot from inside treatment 3, that cord has vanished.
Other mistake: Just as Squad 51 reaches the Tank Motorway, in the shot of the squad as it drives towards the camera before it zooms out, look at the squad's license plate before it stops. Even through the haze, we can see that the plate number is not Squad 51's normal E999007.
Visible crew/equipment: At the horse ranch, after Bill falls off the horse again while the squad is chasing after him, when Roy kneels beside Bill we can see the shadow being cast by production equipment, presumably the reflector screen, on the ground behind Roy. Then after John checks Bill's ears and pulse, just as he contacts Rampart for the second time, we see movement which looks like a sleeve on the right side of the screen, beside John.
The Promise - S3-E11
Continuity mistake: When Roy and John walk into Edna Self's house, in her closeups she's wearing a light beige floral robe, but in the shots facing the guys we can see that it's a dark solid color robe that's on the chair. Notice how odd it is that the three are not in any shots together - Edna's not in any shots facing John and Roy, and they're not in any shots facing her.
Other mistake: When Roy and John are examining the boy who jumped from the ledge, they're standing at the back of Engine 85 while the boy's sitting on the tailboard, and we can see behind his head the vinyl material with the imprinted 85, covering the attached plate with the real company number.
Continuity mistake: When Engine 51 gets the run in the middle of the night, the engine company rushes to the rig including Johnny, which leaves Roy standing in the dorm alone. After he shouts out to Johnny from the doorway, when Roy turns to go back into the dorm, some changes have occurred - the locker room door is now closed, there are clothes lying on Cap's bed where none were a moment ago, etc.
Visible crew/equipment: After Roy tells Jason that he has time to pop up to the orthopedic ward because it's Johnny's turn to cook, when Roy heads out receiving we can see the top of the set over treatment 1, right behind Johnny.
Paper Work - S6-E11
Visible crew/equipment: At the warehouse, when the foreman yells at Ed for stacking the flammable nitromethane steel drums unsafely, the outline of the mic transmitter is visible on the front left side of Ed's T-shirt.
