Best drama TV mistakes of all time
Revealing mistake: At the end of the episode when Tuco shows up at Walter's house in the backseat of Jesse's car, he tells Walter to get in. As they drive off, the shot pans out to show the street sign across from Walter's house. The sign says Piermont, which is the real name of the street. In the show, the Whites live on Negra Arroyo Lane.
Visible crew/equipment: Once the ledge rescue is over, after Johnny tells Brackett that he could put in a good word for him to join the paramedic program, when Roy opens the squad door to get in, the wing mirror offers up nice reflections of a few crew members.

Continuity mistake: In this episode Pam and Jim are talking about how Jim used to have a crush on Pam when she first started here. This implies that Jim was working here when Pam was hired. However, in 4-3: Launch Party Pam says that when Jim first started working there she said, "Enjoy this moment, because you're never going to back to this time before you met your desk-mate, Dwight." These two scenes contradict each other.
Character mistake: Season 3, episode 12, "Mr. Monk Gets Cabin Fever": When Randy Disher is having Chinese food in his apartment with his new girlfriend, he gets a fortune stating that he is going to get money from his uncle. Randy says that he doesn't have an uncle. In Season 5, episode 14, "Mr. Monk Visits a Farm", Randy inherits a farm from his uncle who appears to have committed suicide, but was actually murdered.
Continuity mistake: In the scene where William Fichtner is shown first leaving his trailer and picking up litter in the amusement park, his gloved hand switches from his right hand to his left then back to his right again before he starts talking to the couple.

Continuity mistake: When Shepherd Book tells everyone about the electrical nets, Jayne says, "One day you're gonna tell us all how a preacher knows so damn much about crime." Shepherd then turns to leave. But in the immediate close up of him and Mal, he turns to leave again.
Plot hole: The very ending scene where Peter (in this version died in the ice as a boy so Walter's wife never met him) takes off some of the dust covers then picks up a toy airplane. The toy airplane he got as a child from ... Walter's wife when she was trying to placate him in an earlier episode for trying to run "home". This toy airplane wouldn't have been his as a child in this version of the universe.
Continuity mistake: Before the Roadhouse fire, Ash and Dean are talking on the phone. Ash looks at his watch, on his left hand, it says 8:10, and has a black dial/face. As Dean and Bobby go through the rubble, Dean identifies Ash by his watch. Still on his left hand, it is now upside down, evident by the crown of the watch (used to move the hands of the watch) which previously facing his hand, is now facing his elbow. Also, the dial/face of the watch is now silver instead of black. This is a different watch, and was inadvertently put on the burnt arm upside down.
Factual error: Tiffany asks a guard where he would go if he could travel back in time. He says he would return to 1999 to attend a Judas Priest concert he missed. He says he wasn't able to get close to Rob Halford. The problem is Rob Halford was not the lead singer of Judas Priest in 1999. Tim 'Ripper' Owens was.
Continuity mistake: Willow is turned into a ghost. Throughout the entire episode, she can't touch anything (can't turn the pages of a book, etc.), yet towards the end when she and Giles go to Ethan's shop, and she leaves, you can hear that she opened and closed the door, even though she can walk through walls and can't actually touch the door. The curtain moves when she leaves too.
Visible crew/equipment: When Mulder and Scully enter the abandoned mine through a doorway, the viewer can see at right in the distance a number of people sitting on a wall, and one crouching in front of the wall with what looks like a dog. They are sitting still, but there is still movement there.
Continuity mistake: Right after Cordelia has her vision, Wesley goes to the whiteboard. In the "Case" column he writes "N.D.U.O." underneath what looks like "CARYNSS" crossed out. But once they find out that it is a Prio Motu demon and Angel writes it on the whiteboard, "N.D.U.O." disappears even though Angel did not rub it off.
