Visible crew/equipment: After Johnny rappels down alone, so he can inform the sheriff of the situation with the injured climbers, when he talks to the sheriff beside the squad car, the reflections of the camera/dolly/crane are visible on the chrome corner of the sheriff's car.
Continuity mistake: When Roy and Johnny are on the Coast Guard helicopter, responding to The Boat wreck with injuries off Catalina, there are a few interior shots of them that are flipped, while they're looking out the windows. The paramedic patches are on their left sleeves, but in the flipped shots the paramedic patches are on their right sleeves.
Revealing mistake: When the Bengal tiger is on the roof, just after the officer warns, "If that cat tries to leave this roof, he's dead before he hits the street," there is a closeup of the tiger and the empty landing below the billboard. A few shots later, after Les steps on the glass, there are pieces of meat scattered on the landing below the billboard, so the trained tiger will jump onto it.
I'll Fix It - S4-E2
Continuity mistake: When Station 51 is at the scene of the oil well eruption, underneath a house which was built atop an abandoned oil field, just after Johnny tells Roy that he thinks the whole foundation is slipping, it cuts to a shot of the front porch as one of the porch columns (and part of another) breaks and falls. However, when the guys finally free the trapped man and carry him out, all the columns are upright once again.
Other mistake: When the dispatcher sends Station 51 to 26309 Barcelona Drive, as the squad pulls up in front of the house, the number on the mailbox post reads 4050.
Visible crew/equipment: After they finally get an address for the attempted suicide Squad 51 takes off, and when Engine 51 pulls away from the curb, the shadow of cinematographic equipment is visible on the engine's front, and the reflection of lighting equipment is visible on the side of the engine.
Continuity mistake: When Roy tells Johnny to call Barbara, who heard something that he didn't even say at the movies, in the view from the kitchen Engine 51 is not in the apparatus bay which means everyone else is on a run, but when dispatch drops tones we hear Captain Stanley respond KMG365, and then see the engine parked in the bay.
The Promise - S3-E11
Continuity mistake: When Roy and Dr. Brackett are on 127's Snorkel while Johnny is trying to subdue Richard on the ledge, Brackett is not wearing any rings, but after loading Richard into the Stokes, when Brackett is grabbing hold of the basket in the closeup he's wearing two rings on his left hand, which vanish in following shots.
Other mistake: When Roy and Johnny are with the guy who somersaulted off the roof into the pool, Johnny is in the pool giving him oxygen, and in the closeups Johnny's left hand varies between very hairy, not hairy at all, normal fingernails and bitten up fingernails. Additionally, the hand holding the mask on the boy's face changes back and forth between Johnny's left and right hand, between shots.
Other mistake: When Station 51 is dispatched to the industrial fire, once they arrive there are a few shots of the crowd of bystanders, many in their robes. This footage is from the first episode "The Wedsworth-Townsend Act", during the factory fire at the start of the episode.
Visible crew/equipment: When Roy and Johnny are with the teenage girl who OD'd, Roy walks over to the bystanders, and when he speaks to the mother and daughter, the perpendicular actors' marks are visible on the ground where they stand.
Continuity mistake: When the guys are dispatched to the cardiac patient stuck in the elevator, Gage leaves the mop and bucket in the middle of the floor in the apparatus bay and rushes to the squad, but when they roll out the bay floor is clear.
Continuity mistake: When Roy and Johnny get to the airport and start talking to the boy flying the plane, the clock reads 11:55, but when Frankie puts the oxygen mask on his father it's 12:15, then when Roy reassures the boy about flying straight and level, it's 12:40, and so on, even though it's really supposed to be only a few minutes between these shots.
Visible crew/equipment: At the skating rink, when the guys start walking on the ice and go to the two clowns, the outlines of the mic transmitters and wires are visible on Johnny's shirt, as well as on the back of the blonde skater's green outfit. Then, when the ambulance attendants are walking on the ice, the outline of the lead attendant's mic transmitter is visible on the front of his shirt.
Visible crew/equipment: When Engine 51 arrives at the scrap yard fire, just before Captain Stanley opens his door and steps out in the closeup, the reflection of a crew member/cameraman, wearing a white, short-sleeved shirt is visible on the engine's red surface.
Other mistake: Brackett has Johnny and Roy in his office to listen to recordings on two cassette tapes - the first of 51's transmission with Rampart, and the second of 49's transmission with Pediatric Clinic. Brackett plays the first tape, pops it out and then plays the second tape. The problem is that when Brackett puts the first cassette tape back in the player, it's been rewound, even though Brackett never actually rewound the tape.
Visible crew/equipment: After Captain Stanley and Roy get the trucker out of the wrecked cab, when Stanley takes his pulse, the outlines of the lav mic transmitter and wire is visible on the back of his shirt.
The Wedsworth-Townsend Act - S1-E1
Continuity mistake: When Dixie and Johnny reach the overturned car at the cliff, the bumper on the car goes from shiny to mud encrusted, and the amount of branches, etc that covered the car decreases significantly, by the time the car falls over the cliff.
Visible crew/equipment: After spending all night on the job, when the tired guys of 51 are at the basketball game, the outlines of the mic transmitters and wires are visible on the backs and sides of 51's jerseys, particularly when Roy gets injured. (00:36:00)
Continuity mistake: Having trouble sleeping, Roy gets up in the middle of the night at the station, and when he puts on his turnout trousers his right suspender becomes unfastened at the front, and hangs down the back of his leg when he walks into the bathroom. However, in the next shot as he enters the bathroom the suspender is properly fastened.

Correct, it was J. Pat O'Malley...he also played the grandfather with his grandson when their rocket exploded, and also played "Old Bill" in the episode with Ann Prentiss, where Gage saves the little girl from the burning tree house, and her mother falls in love with him.
