Visible crew/equipment: When the two ambulance attendants remove and roll the stretcher away from the ambulance, towards the stockbroker with chest pains, just as the words "Directed by Dennis Donnelly" appear onscreen, the moving shadow of the cameraman holding a handheld camera is visible on the ground.
Visible crew/equipment: At Rampart, Johnny and Roy step out of the elevator and walk down the hallway discussing the inheritance, and when Roy bends down at the water fountain the outlines of the lav mic transmitter, wire and even the dark strap (that holds the transmitter) around his waist are visible on the back of his shirt.
Other mistake: During the paint factory fire incident, when the Battalion Chief (Battalion 14) contacts LA dispatch over the HT he says, "LA, Battalion 14, this fire is contained," but the bizarre subtitles read "Calling Battalion 14..." Subtitles from Universal Studios 2010 DVD.
Visible crew/equipment: When Engine 51 rolls up on scene at the paint factory structure fire, Captain Stanley heads towards the street corner, and we can see the actor's white T-mark on the ground before he gets on the HT.
Visible crew/equipment: When Roy tells Billy not to get out of the car because of the downed electrical wires, the outlines of the mic transmitter and wire are visible on the back of Roy's shirt.
Visible crew/equipment: When Brackett tells the orderlies to take the stock broker to the CCU and they tend to him, an actor's T-mark is visible under the gurney while it's still propped up, just before Dixie stands on that spot.
Other mistake: During the paint factory fire, when Engine 2 rolls up, we can see the red, large, rectangular vinyl material that is adhered to the front of the engine, with the company number printed on it. There's also vinyl material adhered to the top of the cab as well as the back of the engine with nothing on it, presumably covering the engine company's real number.
Other mistake: When Squad 51 is dispatched to the possible heart attack, while Chet’s at the base station responding to the dispatcher, the lightbar atop Squad 51 has nothing covering the chrome, but en route in the close up there's black tape covering much of the chrome trim.
Continuity mistake: After the paint factory fire, when Roy and John are with the lawyer in Captain Stanley's office, Roy's chair is either directly in front of the desk or off to the side, depending On Camera angles. Their body positions change between shots as well.
Other mistake: During the fire at the paint factory, just after the barrels at the back of the pickup up truck explode, in the next shot we see Marco and Chet manning a hose. In this closeup, another fireman runs up to them and he's wearing a helmet with 51 on it, but it's obvious he's not one of 51's guys, plus Roy and John are with the burn victim and Mike's at the engine. The fact that it's a closeup and we can see perfectly well that he's not actually from 51 makes this too odd.
Other mistake: After Mr. Winthrop is brought to the ER, there is a closeup of his EKG monitor as it's picking up multiple PVCs, but when Brackett asks him about the tension in his life, the monitor seen behind him is showing a flat line. This happens once again after Roy and John leave the treatment room, just before they move him to CCU.
Visible crew/equipment: When 51's guys are sitting around the table discussing the inheritance, numerous overhead set lights are reflected on the chrome coffee pot's surface.
Continuity mistake: When Dixie opens the door to Brackett's office, there's a pile of white papers on the desk in front of Brackett, but when he walks out of the office, in the view from the hallway, the desk lamp has moved and there are no papers where there were in the previous shot.
Visible crew/equipment: After Johnny tells Roy not to phone Joanne about the inheritance, when Chet walks into the kitchen announcing that it'll cost $2.25, the small black tape mark is visible on the floor where Johnny and Roy were standing.
Answer: It is standard practice to wet down driveways so that they stand out in a long shot.
