Mummy on the Orient Express - S8-E8

New this month Continuity mistake: When the Doctor phones Clara and discovers she's trapped in the locked car with Maisie, his phone keeps switching hands.

Flatline - S8-E9

New this month Factual error: After PC Forrest is killed by the Boneless, her nervous system is flattened and displayed on the wall, appearing as a mural. However, there are far too many nerves around where her liver would have been and not nearly enough around her brain.

The Caretaker - S8-E6

New this month Continuity mistake: When Courtney comes into the caretaker's room in search of paper towels, there is a "keep out" sign (reading "Go Away Humans") hanging in the window of the inner door. When the Doctor is talking to her just before she leaves, the sign is missing. Over the course of the rest of the episode, the sign proceeds to appear and disappear a few more times.

Time Heist - S8-E5

New this month Continuity mistake: Near the end, when the Doctor and Clara are standing by the TARDIS on the planet they took the Teller and its mate to, Clara's hands go from touching the buttons on her jacket to loosely clasped lower down. In the third shot of her, they return to the first position.

Death in Heaven - S8-E12

New this month Revealing mistake: During the sequence where the Cyber-clouds covering Earth are burned away, there is a shot showing the clouds burning over the Sydney Opera House. The shot is, very obviously, a still image to which CGI of the burning sky has been added, resulting in an extremely unconvincing image.

The Day of the Doctor - S7-E16

New this month Revealing mistake: When Osgood tells Kate that they're using Derren Brown as the explanation for flying the TARDIS into Trafalgar Square by helicopter, the two women are standing in front of the TARDIS. Its door is slightly open, revealing an interior that is obviously a picture.

The Day of the Doctor - S7-E16

New this month Continuity mistake: After Clara enters the Doctors' cell, and they're talking about how the door was unlocked, shots inside the cell show the door about halfway closed, while a shot from outside looking into the cell shows the door wide open.

The Day of the Doctor - S7-E16

New this month Continuity mistake: During the scene in the barn where the three Doctors have their hands on the button, when Eleven notices Clara crying and asks her what's wrong, the Doctors' hands go from stacked on top of each other on the button to only touching it with their fingers.

The Name of the Doctor - S7-E15

New this month Continuity mistake: When the Doctor is talking to River just before entering his own timeline to rescue Clara, in the shots facing him, the glowing column representing his timeline is not visible, despite the fact that a wide shot reveals he's standing right in front of it.

The Runaway Bride - S3-E14

New this month Factual error: During the chase on the motorway, many deciduous trees sporting lush green leaves can be seen in the background, in sharp defiance of the fact that it's supposed to be Christmas Eve and their branches should be bare. In addition, despite having chosen to wear a sleeveless wedding dress, Donna is remarkably unbothered by the fact that it should be fairly brisk outside, probably somewhere around freezing.

Journey to the Centre of the TARDIS - S7-E12

New this month Continuity mistake: When the Doctor is asking Clara if she feels safe in the last scene, most of her hair is draped over her left shoulder. However, whether some of her hair is draped over her right shoulder or not changes between shots.

The Crimson Horror - S7-E13

New this month Other mistake: When Mr. Thursday goes to see Vastra, to hire her to investigate his brother's death, he doesn't see her face (she's a Silurian "lizard woman from the dawn of time") until she lifts her veil to take a closer look at a photograph he brought her, upon which he faints. However, Vastra's veil is quite see-through, enough that her non-human appearance is obvious to the audience even before she lifts it. The same thing happens when she goes to visit the coroner later - although he doesn't faint.

Journey to the Centre of the TARDIS - S7-E12

New this month Continuity mistake: Near the end, when the Doctor picks up the remote control and reads the words written on it, the position of his hands as he's holding the device changes between the close-up and the wide shot.

Hide - S7-E11

New this month Continuity mistake: When Clara draws the Doctor's attention to the vision of a forest appearing in the archway, the candelabra she is holding has two candles in it, with the third hole empty. Moments later, after the apparition has disappeared, there are now three candles in it.

Hide - S7-E11

New this month Continuity mistake: For most of the episode, there is a thunderstorm going on at Caliburn House. However, when the TARDIS is seen dematerializing and rematerializing when the Doctor and Clara go to take pictures of the ghost throughout time, it's dry in the grove where it was parked. The first time is especially noticeable, as moments later we are shown Professor Palmer and Emma looking through a rain-spattered window, commenting on the noise of the TARDIS' engines.

Deep Breath - S8-E1

New this month Revealing mistake: After the clockwork droids shut down, when the ones in the restaurant basement start falling over, the extended blade of one of the toppling droids flops about like rubber.

Add time

Evolution of the Daleks (2) - S3-E5

New this month Other mistake: After the hybrid Dalek Sec explains the Daleks' plan to harness the power of a solar flare to the Doctor, when the Doctor asks how many minutes until the flare arrives, Sec says it's in 11 minutes. A fair amount of action and backstabbing later, when the Doctor and Laszlo arrive at the top floor of the Empire State Building and after the Daleks take back control of the elevator, the Doctor says that the solar flare arrives in 6 minutes. It is a considerable stretch for everything that happens between those two moments to take place in only five minutes.

The Shakespeare Code - S3-E2

New this month Factual error: The establishing shot of Bedlam Hospital shows it with 18th-century neoclassical architecture, in an episode set in 1599, in the Elizabethan era.

The Shakespeare Code - S3-E2

New this month Character mistake: After the Carrionites kill Lynley, the Master of the Revels, by using the doll to make him drown on dry land, the Doctor says, while examining the body, that he's never seen a death like it. In the Third Doctor serial "The Mind of Evil", involving a monster that could make people relive their worst fears, a character died in exactly that fashion.

Quotes

Doctor: So, that's the trap. Or the test or the final judgment, I don't know. But if I kill you, I kill her. Except that implies, in this big grand scheme of Gods and Devils, that she's just a victim. But I've seen a lot of this universe. I've seen fake gods and bad gods and demi-gods and would-be gods - out of all that - out of that whole pantheon - if I believe in one thing... Just one thing... I believe in her.

Trivia

"Torchwood" is an anagram for Doctor Who. Russell T Davies came up with it during the filming of Series 1 to label the tapes in order to prevent theft and potential leakage, and decided to use the name in the show proper.
