The Caretaker - S8-E6

New this month Continuity mistake: When Courtney comes into the caretaker's room in search of paper towels, there is a "keep out" sign (reading "Go Away Humans") hanging in the window of the inner door. When the Doctor is talking to her just before she leaves, the sign is missing. Over the course of the rest of the episode, the sign proceeds to appear and disappear a few more times.

Time Heist - S8-E5

New this month Continuity mistake: Near the end, when the Doctor and Clara are standing by the TARDIS on the planet they took the Teller and its mate to, Clara's hands go from touching the buttons on her jacket to loosely clasped lower down. In the third shot of her, they return to the first position.

Death in Heaven - S8-E12

New this month Revealing mistake: During the sequence where the Cyber-clouds covering Earth are burned away, there is a shot showing the clouds burning over the Sydney Opera House. The shot is, very obviously, a still image to which CGI of the burning sky has been added, resulting in an extremely unconvincing image.

Listen - S8-E4

Continuity mistake: After the Doctor is knocked unconscious, Clara uses the psychic circuitry to pilot the TARDIS to what turns out to be a moment in the Doctor's past, in the barn first seen in "The Day of the Doctor." The loft in the barn where the young First Doctor is sleeping in bed, however, was not present in the previous episode.

Suggested correction: These events take place thousands of years apart. Buildings (especially wooden ones) are likely to change in that time.

The barn appears three times, in "The Day of the Doctor", "Listen" and "Hell Bent." Its appearance in "The Day of the Doctor" is chronologically in-between its appearances in "Listen" and "Hell Bent." Both of the two latter episodes have the loft, but "The Day of the Doctor" doesn't. That's the mistake here.

Into the Dalek - S8-E2

Continuity mistake: Throughout the episode, the collar of the Doctor's shirt frequently tucks in and out of his coat from shot to shot. Especially noticeable in the scenes where they are looking for and subsequently fix the Dalek's radiation leak.

Kill the Moon - S8-E7

Factual error: Near the end of the episode, the full Moon is shown in the daytime sky, well above the horizon. This is impossible, as any celestial body lit by the Sun has its full phase only when it is directly opposite the Sun; thus, a full Moon rises at sunset and sits high in the sky only during the night. For the Moon to be in the sky during the day as shown, it would have to be visibly of a phase other than full.

Dark Water - S8-E11

Plot hole: When the Doctor and Missy, the latest incarnation of the Master, meet for the first time, she passes herself off as an android. Only later in the episode, after seeing the Gallifreyan hard drive that she has been uploading human minds to, does the Doctor realise that she is a Time Lady, and he doesn't realise that she's the Master until she tells him in the episode's cliffhanger ending. The problem here is that over the show's long history, Time Lords have been shown to have the ability to recognize each other instantly, an ability that makes perfect sense given their powers of regeneration. Previously in the new series, it was even stated that Time Lords can psychically sense each other, an ability demonstrated on-screen in "The End of Time." In "Dark Water", this does not happen, at all. Now, the Master has a history of using technology to block his/her psychic signal before - the Archangel satellite network from Series 3. However, at no point in this episode or the next one, "Death in Heaven", does either the Doctor or the Master even mention such technology as being used, leaving the mystery of how the Doctor didn't recognize Missy as a Time Lady at the very least a rather glaring hole.

Doctor: So, that's the trap. Or the test or the final judgment, I don't know. But if I kill you, I kill her. Except that implies, in this big grand scheme of Gods and Devils, that she's just a victim. But I've seen a lot of this universe. I've seen fake gods and bad gods and demi-gods and would-be gods - out of all that - out of that whole pantheon - if I believe in one thing... Just one thing... I believe in her.

Trivia

"Torchwood" is an anagram for Doctor Who. Russell T Davies came up with it during the filming of Series 1 to label the tapes in order to prevent theft and potential leakage, and decided to use the name in the show proper.
