Factual error: On 4/5/07, Chad had a flashback to when he met Vincent at a gay bar, over a year earlier. The song playing in the bar was Pink's "U + Ur Hand," which came from a CD that wasn't released until months after the scene took place. That particular song wasn't released as a single until about an additional year later.

Continuity mistake: In this episode, Leo tells Phoebe that white lighters are sent to their charges by "a group of elder white lighters known as the founders." However, by later seasons in the series, the word "founders" was never used. The leaders of the white lighters were simply known as the "elders." (00:14:15)

Plot hole: When J.T. Cornell's dead body fell out of the wedding cake on 12/28/06, Rebecca fell all over him, lamenting loudly to everyone about how much she loved him, how much he meant to her, and how amazing the sex was. (She was really just looking for his flash drive.) However, Rebecca and Gwen had spent the entire fall trying to prevent Ethan from catching Rebecca and J.T. together, because Ethan was convinced that if Rebecca and J.T. knew each other, then it would mean Rebecca and Gwen were lying about the tabloid, which they were. So Rebecca's behavior should have been a huge tipoff to Ethan, but instead he had no reaction at all to this.

Continuity mistake: Early in 2006, when she was planning to become a nun, Whitney whipped herself on the back to punish herself for her sins, injuring herself badly. In December of that year, she got married and wore a gown that revealed much of her back, but there were no scars at all.

Factual error: Jerry says that Flynn argued with Stephen Hawking regarding the thesis of Hawking's book when Flynn was only nine years old. However, Flynn celebrates his 32nd birthday in the movie in 2006, meaning he'd have been 9 in 1983 and 1984, while Hawking's first book, A Brief History of Time, wasn't published until 1988.

Revealing mistake: On 10/9/06, John entered the storage facility and reached towards the light cord. He formed a fist, as if grasping the cord, but his hand was actually several inches from it. It was clear that he was not actually holding the cord at all, but then he pulled his fist down and the lights went on.

Continuity mistake: On 7/4/06, Kayla has a flashback to what she says is her and Steve's first Fourth of July with their daughter Stephanie. That would have been July 4, 1990. They talk about the various characters they're going to see at the barbecue later, including Jack. But Jack and the others they mention were actually stranded on a deserted island during the summer of 1990.

The Impossible Planet (1) - S2-E11

Factual error: Ida, the science officer, says that the planet is in geostationary orbit around the black hole. However, the word "geostationary" applies very specifically to objects orbiting the planet Earth. Since a black hole is a type of star, this planet's orbit could be described as astrostationary, or even just stationary, but definitely not as geostationary. A science expert on an interstellar mission wouldn't make this mistake, and she wasn't dumbing things down, either, since "geostationary orbit" is already a pretty obscure topic for people unfamiliar with space technology. Revealing mistake: Throughout the series, Finola Hughes appeared as Patty, the deceased mother of the Charmed Ones. Although Patty died in 1978, she sometimes appeared in flashbacks and time-travel episodes that took place in the mid to late 1970s, and other times she appeared as a ghost. However, over the eight years of the series, Finola Hughes aged noticeably. Patty was born in 1950, so she should always have been either a young woman in her mid 20s, or an unaging ghost having died at 28.

Factual error: In the spring of 2006, Whitney traveled from Harmony to Rome, and we are left to assume that she flew. After Whitney had arrived in Rome, Chad and Simone found out and went to Rome to find her. Paloma and Jessica accompanied Simone. After Paloma arrived in Rome and got into some trouble, Theresa and Ethan found out about it and went to Rome as well. So there were three consecutive trips from Harmony to Rome, and they all happened during the same night; Whitney left after dark, and Theresa and Ethan arrived in Rome the following morning. But each flight to Rome should take six hours, and there is a six hour time difference, which adds up to a full twenty-four hours. So nobody who left Harmony at night could have arrived in Rome before morning. And Theresa and Ethan should have arrived in Rome after dark on the second day, not first thing in the morning.

Factual error: On 5/18/06, it was simultaneously breakfast time in Rome, Italy, and in Harmony, Maine. But since Rome time is six hours later than Harmony time, this is impossible. Breakfast time in Harmony is lunch time in Rome, and breakfast time in Rome is the middle of the night in Harmony.

Continuity mistake: On 9/27/99, in LA, Henry gives Chad a newspaper that was left in his family's apartment when Chad was abandoned at age eight. It is a Harmony Herald. Then we see Theresa and Whitney walking in front of a Harmony Herald newspaper box in Harmony, and the paper is identical to the one that Chad's family left behind about a decade earlier.

Plot hole: In April of 2006, Whitney disappeared from the convent very late at night. Later that same night, it was discovered that she was in Rome. But we never saw her get on a plane - only walking through catacombs with the monk the whole time - and even if she did get on a plane, there hadn't been nearly enough time for her to get to Rome.

Continuity mistake: At the end of the episode on 4/10/06, Billie realized that the trapped girl's cell phone was Chelsea's, and at the same time Bo went into the sewer and discovered that the girl was Chelsea. At the start of the episode on 4/11/06, Billie yelled down to Bo that the girl was Chelsea as soon as she realizes it, and Bo then continues into the sewer, already knowing that the girl is Chelsea before he reaches her.

Continuity mistake: On 3/17/06, Sam and Ivy had a conversation about Kay that was largely at odds with previous events and information from the show. In particular, they talk about how Kay pursued Miguel from a very early age. However, when the show began in 1999, Kay and Simone were in high school and their dialogue established in the very first episode that Kay had only very recently developed a romantic interest in Miguel. Furthermore, most of Harmony, including Sam and Miguel himself, didn't learn of Kay's interest until several years after that.

Plot hole: In the early months of the series, in the summer of 1999, Father Lonigan met many times with his longtime parishioner Grace Bennett, and a woman named Faith Standish who made several visits to town. Grace and Faith were identical twins who had been separated years earlier. Father Lonigan is blind, and usually recognizes his parishioners by their voices, yet he never noticed that Faith had Grace's exact voice.
