Factual error: When Shawn is talking to Belle over the phone, the Eiffel Tower can be seen outside her window, yet, she's supposed to be in Milan, Italy.
Continuity mistake: Oct 2003 - When Belle and Shawn are looking through the photo album, there's a picture of Jen in a wedding dress, and Jack dressed as a fireman. Shawn mentions that was Jack and Jen's first wedding. WRONG. That is when Jack stopped Jen's wedding from Emilio. He faked a fire and kidnapped her. Jennifer and Jack got married at the 'Wild West' arena.
Factual error: Abe Carver was a police commander. Consequently, his funeral should have been attended by the city's entire police force, in full uniform. The funeral would have been a solemn affair carried out in conjunction with the municipal government, and certainly would not have been combined with the funerals of two other people.
Continuity mistake: Shawn Douglas Brady was a ring-bearer, about 4 years old, at the wedding of John Black and Isabella Toscano in 1992. At the wedding, Isabella gave birth to their son Brady. All of the children on the show were recast in 1999 to be much older. Brady Black is now three years older than Shawn, instead of the other way around.
Plot hole: After Rex learned that he was not really a DiMera but in fact a Brady, he suddenly became "poor." But a lot of things don't add up. Firstly, after Tony died, there was never a funeral or a will reading shown, but it seems likely that the twins would have been left something. Secondly, Rex graduated from college in the spring of 2003 and is an incredible genius who invented a laser that turns coal into diamonds. So although he might be on his own financially, he should be able to get a pretty good job without much trouble, but there's no indication that he has even tried. But Rex being poor has instead become a significant plot device, because Bonnie does not want him to marry her daughter Mimi as a direct result of it.
Other mistake: When Billie visited Georgia's grave in Salem, she acted as if the baby was buried there, yet, Billie should know that the coffin buried is just a bunch of rocks and the real Georgia is buried where Earl, Wayne, and their mother live by the bayou. Billie lied to Bo about Hope being the cause of their baby's death and buried the rocks in the coffin to make it look like the baby had died after her fight with Hope, rather than before.
Continuity mistake: The character of Hope Williams Brady was born in real time on January 10, 1974. When the character was believed to be dead in 2000, her tombstone indicated that she had been born in 1963. This was done because the writers had aged the character by about ten years during her absence from the show around 1980.
Continuity mistake: On Christmas, Kate and Roman entered the Brady's celebration and announced that they would be getting married the next day. Sure enough, the next morning, it was to be their wedding day. But as midnight approached after the ceremony, it was New Year's Eve and everybody was getting ready to celebrate the new year, there at the wedding. But it was supposed to be December 26.
Factual error: It is impossible for Jack to donate his organs seeing as he's a cancer survivor.
Plot hole: Whenever John moved at all towards Marlena during the exorcism on 4/20/04, Father Jansen ordered him back right away. But when Marlena calls for Belle and Belle kneels right in front of her mother, Father Jansen doesn't say anything. The only reason is so that the scene's drama can be increased when Marlena begins to strangle Belle.
Continuity mistake: They never do explain why Kate is covered with blood when she comes screaming back into the wedding reception after Roman is killed. One might assume that it was Roman's blood, but if that's the case then why was Roman's body still underneath the table when the others found it? In that position, Kate couldn't have had nearly enough contact with his corpse to get that much blood on her.
Plot hole: On 3/15/05, Jack picks up the phone in his house and calls Jennifer's cell phone. Unfortunately, before he can tell her what's going on, the Jennifer impostor knocks him out and tells Jennifer that it was a wrong number. However, Jennifer's cell phone should have shown her that the call was coming from her own home, which would have been quite suspicious.
Continuity mistake: Caroline Brady's first name had always been pronounced with a long "i" in the last syllable. This is confirmed by the flashback with Victor that was shown on 11/17/03. However, for the last few weeks, since the character returned to the forefront of the story, everybody has inexplicably pronounced the name with a short "i." This includes Victor in the 11/17/03 episode, immediately after the flashback.
Continuity mistake: On 2/24/04, Marlena struck repeatedly with her knife when she stabbed Jan, thinking it was Shawn. But when Jan reached Nicole on 2/26/04, she had only a single stab wound.
Continuity mistake: In December, when Kate and Roman's wedding started, Chloe's hair was crimped and hanging loose behind her head. By January, while the wedding was still going on, Chloe's hair was instead pulled up in a ponytail, and the hanging portion of her hair was straight.
Continuity mistake: When Shawn Douglas Brady was a small child around 1990, he was friends with Sami, Eric, Carrie, and Nikki Alamain, all of whom were about his age. Somewhere along the way, everyone was aged much more than he was, and now he's supposed to have grown up with Belle Black, who was actually born some six years after him. He's in his sophomore year of college, while his aforementioned, erstwhile friends are all in their late twenties.
Plot hole: On April 1, 2004, Marlena fell over the penthouse balcony and landed directly on Sami. The next week, everybody was talking about how Lucas saved Sami's life by pushing her out of the way of Marlena. But this makes no sense, as the collision couldn't possibly have been any worse than it was. So what exactly does everybody think Lucas did?
Continuity mistake: At the very end of the 11/24/03 episode, Marlena remembered that she ate the vegetable dip right before she collapsed; it is poisoned. At that moment, Hope takes some dip on a carrot and walks past Bo as he confronts Rex. She moves the snack to her mouth and the shot freezes. At the beginning of the 11/25/03 episode, the scene is replayed. Marlena again remembers the dip connection and Hope again takes some dip on a carrot. But before she moves anywhere near Bo, Celeste takes her aside to warn her of impending danger.
Plot hole: When Lucas pretended to be the killer on 12/16/03, he said that he wanted to give Sami a scare after becoming suspicious of her when he followed her to his mother's hotel suite. But if he was following her, then how did he get to the bedroom before Sami to jump out at her? And where did he get the cloak and mask on the spur of the moment?
Factual error: The Salem Police Department is grossly misusing the term "serial killer" in conjunction with the Abe Carver murder. The inaccuracies are too numerous to list, but the most obvious is the fact that the police believe that Abe was murdered by someone with a specific motive. A serial killer selects victims based on a general profile, usually in conjunction with a psychological "need" to commit the crime. The police brand the unidentified murder a serial killer based purely on the fact that they think he or she may strike again. But not every multiple murderer is a serial killer, and the police would certainly know better than to fall prey to this misnomer.
