Into the Hornet's Nest - S1-E4
Continuity mistake: When Pegasus is welcoming everyone to the tournament, Yugi looks at his deck. In the shot of him raising the deck, the bottom side of the cards is facing the palm of his hand. But in a close-up of the cards, the bottom side of the cards is facing away from the palm of his hand. (00:05:44)
Into the Hornet's Nest - S1-E4

The Gauntlet Is Thrown - S1-E2
Character mistake: When Pegasus shows Yugi the Faceless Mage card, Yugi says, "But my Dark Magician is one of the strongest magic cards in the game." Dark Magician is a monster card, not a magic card. (00:16:50)
The Heart of the Cards - S1-E1
Continuity mistake: When Solomon wakes up in the hospital after the duel, the elastic string attached to his oxygen mask is at a straight angle. In the next shot, the elastic string is at a tilted angle. (00:19:08)
The Heart of the Cards - S1-E1
Continuity mistake: When Kaiba summons his first Blue-Eyes White Dragon to the field, Joey says, "Impossible. We all watched Kaiba tear that card in half." At this moment, his arms are straight. When the shot cuts, his arms are bent. (00:13:37)

Character mistake: When the duel begins, Kaiba says, "I attack with the Hitotsu-Me Giant," but once the monster is summoned, he doesn't attack Yugi with it. (00:10:20)
The Heart of the Cards - S1-E1
Continuity mistake: When Kaiba holds up the Blue Eyes White Dragon card he took from Solomon, in the shot of him raising the card, he has two fingers pressed against the front side of it. In a close-up of the card, he is holding it with three fingers, and the fingers are at a different angle. (00:07:46)
The Heart of the Cards - S1-E1
Continuity mistake: When the KaibaCorp thugs walk into the card shop, there is a close-up of Solomon where we can see his face is half-covered in sunlight. Then it cuts to the head thug speaking to him, but when it cuts to Solomon saying "and if I were to decline," he is entirely shadowed. (00:06:10)
The Heart of the Cards - S1-E1
Revealing mistake: When Kaiba shows Solomon the briefcase full of cards, there is one card with 1600 attack points and 1800 defence points. The defence points of the card appear to be a lot lighter than the card's attack points, as well as the attack and defence points of the other cards. (00:04:52)
The Heart of the Cards - S1-E1
Revealing mistake: When Yugi and Joey are duelling at the start, in the shot of Tristan approaching Joey, the cards that Yugi is holding appear to be drawn incorrectly. The cards don't have the text or illustrations that are usually shown on them. (00:01:46)
Champion vs. Creator, Part II - S1-E27
Character mistake: Pegasus says his toon world cannot be penetrated, but that is not true since it's a magic card, so any card that destroys magic cards can destroy it. Furthermore, monster special effects can also destroy it.
Rise of the Great Beast: Part 2 - S4-E40
Continuity mistake: A baseboard can be seen on the wall behind Alister's hospital bed in the shot of him sitting up that wasn't there in the establishing shot of the room. (00:11:41)

Continuity mistake: The nightstand next to Alister's hospital bed has three drawers on it. The third drawer at the bottom of the nightstand is larger than the two drawers above it. A few seconds later, all three drawers are the same size. (00:11:42)
Continuity mistake: After the robot summons Blue-Eyes White Dragon, it ends its turn after seeing Kaiba's face-down card. In the shot where the robot says "End turn," the cards on Kaiba's duel disk are missing the illustrations. (00:10:29)
Winning Through Intimidation - S1-E15
Deliberate mistake: In the actual card game, attacking the castle ring would not be allowed, but this is probably done in order to enhance the show for kids.
