Visible crew/equipment: When Stark and Aeryn are back aboard Talyn with Xhalax, who is tied up, when Stark says, "As far as I can tell, Talyn's systems are almost back to normal," we can see the puppeteer's face (wearing a weird black mask) and hands under Rygel's hoverchair. Funny.

Continuity mistake: Willow is turned into a ghost. Throughout the entire episode, she can't touch anything (can't turn the pages of a book, etc.), yet towards the end when she and Giles go to Ethan's shop, and she leaves, you can hear that she opened and closed the door, even though she can walk through walls and can't actually touch the door. The curtain moves when she leaves too.
Continuity mistake: Right after Cordelia has her vision, Wesley goes to the whiteboard. In the "Case" column he writes "N.D.U.O." underneath what looks like "CARYNSS" crossed out. But once they find out that it is a Prio Motu demon and Angel writes it on the whiteboard, "N.D.U.O." disappears even though Angel did not rub it off.
Continuity mistake: Up until Green No More in Series 2 (where the Green Ranger powers are destroyed), the Green Ranger's overshield constantly changes between solid-looking metal to flimsy-looking cotton. This is the most obvious difference between the Japanese and US footage.
Continuity mistake: After Mrs. Russo puts Justin and Max in the lair, she is wondering if she should put the big lock on the door or not. She decides not to and puts the lock down on the counter. Two shots later you see the lock is back on the door, but unlocked. You never see Mrs. Russo put the lock on the door.
Visible crew/equipment: When the horse carriage is ambushed, just as Princess Abigail can be heard screaming "Please, help me", the camera pans downward and a horse wrangler wearing a blue baseball cap can be seen in the lower left hand corner, hiding behind one of the horses.
Visible crew/equipment: At the beginning of the episode, Jeannie is talking on the phone by the stairs and her sister is hiding behind the banister. Her sister stands up for a moment and shows her full face, revealing that it is actually Barbara Eden's stand-in wearing the green costume.
Continuity mistake: Ep. "Samson and Delilah" - All shots of the Jeep Cherokee prior to it crashing (and during the process of flipping over) show that it wears factory-optional silver steel wheels of a 'star' style. But as the wreck settles upside-down, it's wearing plain steel wheels (like those found under full wheel covers or on a spare tire).
Other mistake: In the scene immediately after the opening credits, Jim is talking to Melinda on the phone. Right as he's hanging up, someone calls out "Dr. Clancy? Got a minute?", to which Jim waves in acknowledgement and starts up the stairs. (Jim's spirit has been in Sam Lucas' body since season 4 and in an earlier season 5 episode, his white coat is embroidered S. James Lucas, M.D).
