Keith's Machinations, Part 1 - S1-E31

Continuity mistake: Just after Metalzoa is destroyed, when Joey says "Not bad, huh, I only wish you could've seen the look on your face when that Metalzoa of yours gets shish-kebabed," he has three cards in his hand. Then on Joey's next turn, he places a trap card face down, then summons a monster card (both of which were cards from his hand), but when Joey and Keith's life point counts are shown after Keith's next turn, there are at least four cards in Joey's hand. Keith's Machinations, Part 1 - S1-E31

Revealing mistake: When Pegasus starts applauding, everyone turns to face him. There is a shot focusing on Yugi, with the other main characters standing in the background. Their faces appear to be drawn poorly, having little to no facial features, with only a few small dots for eyes. (00:01:21)

Duel Identity, Part I - S1-E29

Other mistake: When Yugi draws Summoned Skull from his deck, the spell card Rose Whip can be seen on the display of Mai's cards on the field in front of him. However, she doesn't activate this card until later in the episode. (00:08:45 - 00:14:00)

Duel Identity, Part II - S1-E30

Character mistake: Yugi mistakenly says if he doesn't get a monster out to defend himself Mai can attack him directly, which wouldn't be possible since the rules of Duel Monsters during the Duelist Kingdom arc forbade direct attacks on defenseless opponents. That rule wouldn't be implemented until the Battle City arc.
