Visible crew/equipment: (Ep. 3) After Omega has revealed himself to The Third Doctor, Jo, and Dr. Tyler, when he begins to speak privately to The Doctor the camera casts a shadow on Omega's body. Then after Omega turns around the boom mic dips into view at the top of the screen. And when Omega starts walking forward, the shadow of equipment (presumably the boom and mic) is moving on the far wall, near the lower left of the screen. (00:03:10)

Revealing mistake: While Julius is talking to the man in front of the lingerie store on Hollywood Blvd, when the man's partner rides up on a motorcycle and tries to snatch the suitcase, we can see the stunt wire carabiner attached to the back of his jacket as he flies off. (00:12:30)

Visible crew/equipment: When Julius enters the supermarket parking lot, in the long shot we can see the stunt ramp used to lift the driver's side of the car, to go into ski mode. In this and following long shots of the stunt, we can also see the window roll cage bar directly behind the front windshield (the obvious stunt driver grips the roll bar with his left hand and steers with his right hand), as well as the top of the seat supports behind the front headrests. In this shot there's a reflection of the camera operator's leg, wearing shorts, white socks and sneakers, and who is sitting where Sandra should be.

During their drive we can see through the window behind them, and there's a crew member wearing a red shirt riding along in the back, as they drive past a parked 1970s station wagon. Visible crew/equipment: When Roy and Johnny are driving in the squad, while discussing Roy's last phone call from Susan, Johnny's lack of tact, etc., and particularly as they're being dispatched to the "possible maternity case," in every shot facing Johnny pay attention to the window behind the guys, because we can see a crew member's right hand, wearing a ring, who's holding a rope while he's riding on the back of the squad.

Then after John pays Ike for the gas, Calvin pulls alongside John just as he reaches up to wipe his truck's windshield, and we can see the bulging outline of the mic transmitter and its wire on John's back.
