Continuity mistake: Part 6, A Desperate Venture: The new Second Elder threatens Carol and has her write a note to John. When Carol hands the finished note to the new Second Elder, this close-up dissolves into another close-up of the note being held by Barbara. Carol's note is quite different in both of these close-ups.

Visible crew/equipment: Part 5, Kidnap: In the Sensorite lab, the Scientist shows the Doctor and Ian a collection of things left behind by the humans, who were killed in the spaceship explosion. When Ian flips through the family snapshots, the shadow of the boom mic passes over Ian's forehead. It happens again just as Susan blurts, "The City Administrator, of course!"

Visible crew/equipment: Part 4, A Race Against Death: While Susan is with Ian and the First Elder, Ian tells her, "The Doctor must have run into a snag..." When Susan replies that she's going to find out why, just as she stands up a boom mic becomes visible at the top right side of the screen, and it's pulled up out of view. Then, as the First Elder says, "I will have you conducted to the laboratory," the shadow of the boom mic is on his head.

Visible crew/equipment: Part 4, A Race Against Death: After the Doctor states he's going off on an expedition, John still struggles to warn the Doctor, and when the Doctor tells Carol, "Now you stay with him, I think he's getting better," a boom mic becomes visible at the top left side of the screen.

Visible crew/equipment: Part 4, A Race Against Death: The Doctor chides the First Elder about Ian's illness and says, "I must have chemicals and equipment; his death will be your fault and yours alone." Then, just as the Second Elder appears and says, "Sir," the boom mic (with white end) swings up between the two Sensorites, and we also see the diagonal boom pole moving at the left side of the screen.

Visible crew/equipment: Episode 1: After Sarah teases the Doctor a bit when he's being somewhat moody, just as the TARDIS is suddenly thrust off its flight path (which was back to UNIT), Sarah and the Doctor are thrown about in the wonky console room (because the camera turns sideways), and the second camera becomes visible at the left side of the screen.

Visible crew/equipment: While The Doctor and Zoe are on the SIDRAT, The Doctor expresses hope to get the hang of its controls. When the SIDRAT materializes, in the shot facing the door as The Doctor pokes his head out to exit, the camera turns toward the right and there's a reflection of the moving camera and the camera operator walking behind the camera, at the right side of the screen. Revealing mistake: Episode 6, while in the barn Von Weich asks Moor (played by David Troughton, son of Patrick, who plays The Doctor) to retrieve the monocle in his pocket. Then, when Von Weich continues with his plan, in his closeups we can see a vertical piece of mesh tape over the actor's right eye. (00:13:05)

Visible crew/equipment: (Episode 5) When Vicki is in the jungle she happens upon the TARDIS and bangs on the door begging to be let in. In the next shot we can see the boom pole/mic hovering in the background, then a hand appears which seems to guide the actress to head in that direction. (00:07:45)

Visible crew/equipment: (Episode 4) After Ian shudders from the apparition he continues to follow The Doctor down the hall, and the camera casts a shadow on Ian. Then, when The Doctor finds the stairs leading to the laboratory he tells Ian he must see what's on the table. Just as Ian states his displeasure at the thought, the boom mic (and its shadow) pops up into view at the bottom of the screen, in front of Ian. (00:05:15)

Continuity mistake: During the animated portions of Shada's missing scenes, in numerous shots an illustrated character's clothing (buttons, scarf, etc) become flipped, sometimes even within the same shot. Example: when The Doctor goes to the physics lab and meets Clare Keightley, note her buttons and The Doctor's scarf.

Visible crew/equipment: After Chris exits the TARDIS with the medical kit, Romana places the collar around the Professor's neck. When Romana tells Chris that Chronotis, "Should be able to think with it," in the next closeup of Chris the moving boom mic is visible at the top of the screen, before he kneels down.
