Continuity mistake: When Joe is dispatched to the domestic disturbance, while he's in the house with the couple Joe's badge quite instantly switches to the opposite side of his shirt, then it's back to its original side. (00:11:10)
Continuity mistake: At the start, while Peter and the kids are watching TV, the buttons on Peter's shirt are on his left side, but in Peter's closeup the buttons switch to his right side, where they usually are. The switching happens again in following shots.
Continuity mistake: While Lois is in the Paris hotel suite packing her suitcase there are two doors behind her, and Bonnie opens one of those doors. The other door has its door knob on the left side of the door, but when Lois walks over and opens that other door to reveal Joe, its door knob is now at the right side of the door. (00:18:00)
Continuity mistake: When Mayor West and Carol are walking on the beach, the buttons on West's suit switch to the opposite side, and this occurs again in later scenes. When Carol and West get engaged, the buttons on Lois' blouse switch sides as well. When Carol's ex-husband, Randall, gives Peter the bag of fish, Randall's buttons also switch from the left to right between shots. (00:06:30)
Continuity mistake: When Carol's taxi stops in front of the house Lois rushes out to welcome her sister, and in the first shot the semaphore flag is attached to the curbside mailbox with its "L" shape facing the ground, but in the next shot as Quagmire shows up, the flag is now attached to the mailbox the opposite way. Continuity mistake: When Lois starts moving Stewie's crib, there's a crescent moon on the headboard with its points facing the left, but in the following shots the crescent moon is facing the opposite way. (00:01:40)

Continuity mistake: During their lightsaber duel, Luke is holding his lightsaber when Vader cuts off his right hand, then Luke's hand and his lightsaber fall away so he loses both. However, after Luke is rescued by the Millennium Falcon, when the door opens as Lando helps an injured Luke, we can see the hilt of Luke's lightsaber hanging from his belt as he hugs Leia. Continuity mistake: After Fett takes off with Han frozen in carbonite C-3PO cracks a joke about rescuing him in three years. When it cuts to Vader with Luke, Vader uses the Force to break a pipe off the wall, and in the next wide shot Vader's chest plate is backwards, note the buttons. (00:35:10)
Episode VI: It's a Trap - S9-E18
Continuity mistake: In S8xE20, "Something, Something, Something, Dark Side," when Han was frozen in carbonite he was smiling with head turned toward his right side, but in this episode, after Jabba watches Luke's message, Han's carbonite is hanging on the wall and now Han is smiling with his head turned toward his left side. Continuity mistake: After Fett flies off with frozen Han it soon cuts to Vader and Luke during their lightsaber duel. When Luke is being pulled through the broken window he's holding his lightsaber in his right hand, then it disappears from his outstretched hand in the wide shot, but in the next closeup his lightsaber is suddenly clipped to his belt at his left side. Continuity mistake: When Mr. Washee Washee walks into the bedroom while pointing a gun at Peter, the buttons on Mr. Washee Washee's shirt keep switching to the opposite side in following shots. (00:05:15)

Continuity mistake: Throughout the episode the buttons on various suits and shirts repeatedly switch sides, between shots. For example, while Brian and Rush are in the RNH building. (00:08:15)
Continuity mistake: When Peter walks into the dry cleaners the door sensors go off, and as Peter leans down to wave his hand in front of a sensor, all the buttons on his shirt switch to the opposite side, then revert back. (00:02:15)

Continuity mistake: When Peter starts trying to say it's OK to swear sometimes, the thermostat dial on the wall behind him, that was under the picture frame has disappeared from where it was. Then when Lois is next to him recalling to him how they met, the dial reappears on the wall. (00:01:10)
Continuity mistake: The buttons on Peter's white shirt switch between the left side and right side, between shots, such as when he tries to play frisbee with James, or when Peter is late for dinner with James.

Continuity mistake: At the hearing, when the middle judge is saying Brian is a danger to society, the woman in blue on the far left folds her arms and places them on the desk. In the next shot, her arms are still folded but now behind the desk. (00:20:26)

Continuity mistake: At the pound, when the keeper says sentences Brian to lethal injection, Lois stands up holding Stewie. Stewie's arms are down at his sides draped over Lois' arms. Continuity mistake: After Brian storms out of the kitchen, Peter is holding a leash in his right hand. When it cuts to a close up of Peter before he says Brian won't get far, suddenly his right hand is empty. (00:12:55)

Continuity mistake: The morning after the dog show, Peter and Chris are watching the news in the kitchen. Peter has a plate in front of him with two eggs and bacon on it. It is turned so that it makes almost a smiley face with the bacon towards his arm and the eggs crooked to the side. In the next shot from a different angle showing Lois in the kitchen, the plate rotates so now the egg and bacon smile is directly facing Peter. (00:10:27)

Continuity mistake: As the cop is telling Peter that leash laws are for Brian's own good, he points his pen at Peter with one hand, holding his clipboard in the other. It cuts to a different angle mid sentence and suddenly the cop is ripping the ticket out of his clipboard and his pen has vanished. (00:09:44)
Answer: Lacey Chabert was the actress who originally voiced Meg in early episodes. She left of her own accord due to being in school and other acting work; Seth MacFarlane has stated there was no tension with her leaving, but it makes for a handy joke for the show.
