Continuity mistake: In Gambit's flashback it is shown that the chosen one from the Guild of Thieves is banished to a "darkness never-ending" for breaking the covenant of the tithe. However, the same character can be seen later in the episode fighting alongside his fellow guild members.

Continuity mistake: In the season 7 episode "I'm Gonna Be Like You, Dad" Alan and Amy's youngest son Joshua is shown to be an infant. However in this episode, which takes place roughly 4 months after the aforementioned episode, he is shown to be around 2 years old.

Hot-Rodding Teenagers from Dimension X - S1-E4

Other mistake: General Traag's weather weapon causes a blizzard across the city, yet when the Turtles and the Neutrinos come up to the surface after the blizzard had been occurring for a while, none of the cars parked on the street, including the Neutrinos' two flying convertibles, are covered in snow. The interior of their cars should have been full of snow.

Audio problem: When Donatello tries to stop April from leaving the sewers after she accuses them of committing the robberies, he is speaking in Raphael's voice. (00:10:35)

What A Drag! - S7-E11

Plot hole: At the beginning of the episode, Eric and Jack call the cops on Luther for planning an illegal rave at the college, but the cops let him go because he had not started the rave. He still broke into the student union building, and therefore should have still been arrested. He even threatens Eric and Jack afterwards right in front of the cops, and they do nothing about it. Revealing mistake: The pitchfork that the Amish girl throws at Eric at the end of the episode is obviously made of rubber, as it wobbles when he catches it.

Other mistake: After Jack and Rachel tell everyone they broke up and Angela says to Shawn "Friends, okay?", the camera is noticeably out of focus and then comes into focus when Shawn says "Hate you or love you..." (00:05:10)

Other mistake: Most if not every time someone kills a walker in close proximity or repeatedly bludgeons one that is already dead with a blunt object and there is a CGI blood spatter effect (as opposed to a practical blood spatter effect), the blood magically never gets on the person's face.

Revealing mistake: When Daryl, Maggie, T-Dog, Glenn and Rick are killing walkers with mêlée weapons on their way inside the prison, there is a random CGI blood spatter that appears on the crossbar of the fence in the foreground. (00:21:25)

Revealing mistake: When Wilson crashes the truck towards the end of the film, you can tell it's actually a stunt driver because he has a full head of hair.

The Last Temptation of Homer - S5-E9

Trivia: At one point in this episode Homer is watching "Eye on Springfield" and Kent Brockman makes reference to Bill Clinton having an extramarital affair - more than 4 years before Clinton's affair with Monica Lewinsky became public knowledge.

Revealing mistake: When Jack and Ennis are cliff diving naked, if you look closely at Jack when they both run and jump off the cliff you'll see that it's not Jake Gyllenhaal but rather his stunt double.

Continuity mistake: The last shot of the plane as it's taking off from the runway with Malloy's car attached to it is a flipped shot. Notice the logo and Jailbird writing near the nose of the plane are backwards.

Factual error: Neither Willie nor the human sacrifice victim show signs of significant perspiration when being lowered to the lava. They should have been sweating profusely after being close to the lava.

Factual error: Indy rescues Willie from the lava pit and immediately releases her from the cage. In reality the cage would be too hot to touch for several minutes after being that close to the lava.

Visible crew/equipment: Crew members can be seen reflected in the rear view mirror of Clay's convertible after he and Blair have their argument in the tunnel.

Revealing mistake: After The Russian smashes The Punisher's refrigerator door against his head a couple times, he shoves The Punisher to the floor and pushes the refrigerator door shut, but the refrigerator door suddenly and completely stops moving midway before closing, revealing the door to have a stopper built in so as to not actually hit the actor in the head with it (acknowledged by the director in the film's commentary track).
