Continuity mistake: Dan throws his umbrella and it bounces on the edge of the wastebasket but lands on the floor. When he walks next to the wastebasket the umbrella is now inside.

Deliberate mistake: When the gangs meet in the toilets to set a date for the brawl, the mirror on the left is the only one opaque, most surely to avoid reflections of the filming crew.

Visible crew/equipment: When Simon and Katya are in the helicopter, both are replaced by obvious stunts. Simon has dyed blond hair as opposed to his brunette stunt, while she has short and straight hair, but her stunt's is long and curly.

Revealing mistake: When Simon is in the helicopter and shoots at McClane, check the metal strips next to his feet and the protruding metal parts where the squibs have been placed to allow for the pyrotechnics. It's particularly noticeable in the aerial shots where they couldn't quite be painted to match the asphalt parking lot.

Continuity mistake: When Miguel is with the pedophile, he is given bread (or something similar) to eat. Shot changes, and the bread is suddenly bitten. The bread's length keeps changing several times between shots.

Continuity mistake: When Merlin arrives from the future, he talks about it to Arthur and puts the crown on his head. The sword, resting against the throne, appears and disappears randomly all the time.

Visible crew/equipment: When Torch is in the Black Forest, watching the crystal ball, 4 big light screens are reflected on it.

Revealing mistake: On the collapsing bridge, when the Phantom saves the kid, they swing on a rope hanging from the middle of the bridge. From a wide angle, it's very obvious that there is no possible way whatsoever that the short rope reaches the safe edge of the cliff. When the angle changes to the edge of the cliff, the rope is suddenly hanging from somewhere right above the edge.

Revealing mistake: When Diana is about to jump off the hydroplane, the safety vest on her back protrudes. Also, a safety bar along the plane's left side has been added for her to grab.

Revealing mistake: The natives use a pair of nooses to trap the bad guys, but when they're lifted up, one can see the protruding hook and mechanism under their shirts.

Revealing mistake: When the Phantom chases the truck, he holds onto a branch where he later stands to attack the truck. In the close-up, one can see it's an obvious metal bar to allow for a safe stunt.

Revealing mistake: When Diana is about to jump off the plane onto the galloping horse, her hair moves wildly. When the shot changes, she has been replaced by a stunt who looks nothing like her and has her hair fixed.

Continuity mistake: Sam points forward to an empty and clean road, to signal her mom where the tornado is. The car keeps driving forward and when the shot changes, cars have appeared next to it and the road is covered with leaves and branches.

Revealing mistake: Before the car pulls up by the diner, after smashing a billboard, skidmarks from previous takes are visible on the ground.

Revealing mistake: When the Overlord extends his arm outside the truck, lights up and electrocutes a cop the shadow that his arm casts is not real, but FX made. Problem is, his arm does not cast any shadow on the truck for a couple of shots.

Visible crew/equipment: When the cop commands Overlord to turn off the engine, a big black tower from the filming equipment is reflected on the front part of the truck. It is not visible in the wide shots of the place.

Visible crew/equipment: When Aunt Boris leaves, a big lighting screen is reflected on the cab's rear window.

Continuity mistake: When Oppenheimer and Kitty are on the ranch, drinking from the flask in turns, he takes a sip. But from a different angle, his hand is suddenly down.

Continuity mistake: When Los Álamos is being built, Oppenheimer talks with a guy and draws four letters and circles on the board behind. Their styles change between takes.

Revealing mistake: When the AA machinegun on the train goes berserk and destroys everything, Indy hides behind some crates. For several shots, Harrison Ford is replaced by a lame CGI, which transforms his head into a videogamish, lifeless face.
