Visible crew/equipment: When Luka and Mark are questioning Amira about the knife Sobriki had used, Kerry walks in then tells Amira to go upstairs to get security to open the personnel office, and the green T-mark taped to the floor is visible near Luka's feet. (00:16:45)
Audio problem: When the team is moving Mr. Metcalf to a different room, Luca says, "Let's go people. Prime the rapid infuser." Abby says, "subclavian on the right." No Good Deed Goes Unpunished - S9-E13
Character mistake: Dr. Corday finishes assessing a gun shot victim, then goes to the next trauma room to assess another. The steady cam follows her. She makes sure not to contaminate her gloves on the door, opening it with her elbows, but then goes straight to touching the next victim. Of course, in real life, she'd change gloves first. The shot never changes, so there's no chance of her changing gloves off camera. (00:12:30)
Continuity mistake: Inside the first 10 minutes, Greene finishes a trauma and his shift and heads upstairs to take Ella from Corday, so she can start her shift. Next, we go back down to the ER, where Benton and Finch are assessing a trauma victim. In the background you see Greene checking an X-ray film, but he has meant to already leave with his baby.
Revealing mistake: As Dr. Benton is leaving the nursery, there are two people who are supposed to be walking down the hall. But, it is very obvious that they are waiting on their mark, waiting for the go ahead before they start walking. (00:38:40)

CaƱon City - S12-E1
Continuity mistake: Sam and Luka are driving to Colorado, presumably in a hurry, to try to find her missing son, Alex. While their driving in the daytime, you can notice inconsistencies in the traffic and road. They are driving on the interstate, but getting passed up by semi trucks. The trucks are not slowly passing them either. It's almost as if they are stopped and the trucks are whirring past them. Then in one shot you see a car change to the left and start to pass, but when the camera angle changes, you see Sam passing up a semi truck at a high rate of speed. A moment later, as their still chatting, an exterior shot of the car shows them changing to the left lane to pass a truck, however the lines in the road are yellow, meaning they are not on the interstate anymore.
Revealing mistake: Neela is delivering a baby. As the camera moves around a nurse to show a close up of Neela and the baby, you see her holding the "head" about a foot away from the mom and quickly moves it closer to look like it's coming out of the woman.
Continuity mistake: Chuny tells Abby that Weaver is looking for her because she didn't call Child Services for a patient. Abby then goes to a phone to call. While she's on the call she hands Malik some sporting event tickets. The next scene shows Weaver out on a date when she bumps into Malik and Susan on their way to the sporting event. The very next scene is back in the ER where Abby is telling Carter that Lewis (Susan) just discharged a patient. The scenes don't line up, as they give the impression that Weaver, Malik and Lewis are all on shift at the hospital, when they're out on the town at the same time.
Homeless for the Holidays - S3-E10
Character mistake: When Doug leaves work, he tells Carol he is Going Home to watch the channel 11 Yule log. Later on, he shows up at her house with Charlie and Carol says, "I thought you were watching the channel 9 Yule log." Continuity mistake: Haleh puts a bow on the top of the organ cooler, the next shot it is on the side.
Continuity mistake: Carol is seen walking into the ER twice at the beginning of the episode.

He is complaining of headaches, Carter and Lucy decide to rule out meningitis and do a lumber puncture. Paul struggles during the procedure and believes that both doctors are stabbing him. Lucy, who is interested in Psych thinks something is wrong with their patient mentally, but Carter is dismissive of her. She gets cross with his attitude and she calls the psych department anyway to have a look at him. Later on and near the closing 20 minutes or so of the episode, all the staff start playing loud music and get a cake which appears to be blue lol, together to party as it's Valentines Day, Carter returns from somewhere and asks where Lucy is. He goes to check one of the rooms for her and Paul Sobriki comes up behind him in the dark and stabs him twice in the lower back, he collapses and as he falls unconscious he sees Lucy on the ground also. In the next episode, Be Still My Heart (ep 14), Dr Weaver discovers them and all of the staff work to try and save them, whilst Carter survives. Lucy's wounds are fatal and suffers a pulmonary embolism as Corday and Romano try to save her. Carter is grief stricken and feels guilty about the way he treated her, he becomes addicted to painkillers and Benton, Greene all help him and book him into rehab.
