It's All in Your Head - S8-E15

Other mistake: In the opening sequence, there is an aerial shot where the camera pans over Central Park in New York City. The trees are full of leaves and some are changing color. The problem is that it is February when this episode is taking place, and the trees would have no leaves left on them.

An Intern's Guide to the Galaxy - S11-E5

Continuity mistake: Near the end, when Ray asks if he can go home, we see Sam writing her number of discharged patients on the board in a close up. The camera then cuts back to the whole group and the size and style of Sam's writing changes.
