Continuity mistake: As Luka talks to the kidney patient, he is holding his chart to his chest. But several times during the scene, when the shot is on the patient, his hands are at his waist, but at other times they are folded in front of him.
Greene with Envy (a.k.a. Lawrence of Northwestern) - S6-E3
Continuity mistake: When the shooter in the ER drops his gun, it falls in front of his foot. Just as the shot changes, however, the gun is to the right of his foot (without him moving) and his foot pushes it aside as he runs away.
Continuity mistake: When Alice opens the back of her van and Carol sees all the dogs, a large spotted dog starts to come towards them - but when the shot changes to from inside the van, that dog is no longer there.
Homeless for the Holidays - S3-E10
Continuity mistake: Nick the dog is standing in the sink and Mark and Doug have got him just about completely covered in soap. The dog jumps out and runs out of the room - by the time he reaches the hall, only a couple of seconds later, the soap has almost completely disappeared.
Stuck on You - S5-E6
Continuity mistake: The patient, Mr. Levy, is talking to Carol and her assistant, and as he says, "I have sugar diabetes, hello!" his left arm goes down to his side. But in the next quick shot, both his arms are folded over his chest under his overcoat.
Continuity mistake: Benton is talking to Carla about her preganancy test and they go through the double doors to the snowy outdoors. Carla has nothing in her hand when she pushes the doors open, but once outside she is holding a hat - she wouldn't have even had time to pull it out of her pocket.
Continuity mistake: When Mark is having breakfast with Rachel and they are talking about skating, her table napkin, which was under her left elbow, falls to the floor. However, the next shot shows it back on the table, with her knife on top of it.
Do One, Teach One, Kill One - S2-E3
Continuity mistake: Near the beginning of the show, Susan is talking to Kerry while holding baby Susie and a stuffed Sylvester the cat; the toy is in a different position in each shot and in the last shot it has disappeared altogether. As Susan walks away, the toy is in the diaper bag, although we never saw her put it there.
Be Patient - S6-E15
Continuity mistake: Near the end of the show, Mark is talking to his father, who is putting his bathrobe on; he has one arm in the sleeve but as Mark says, "I need to talk to you", his father turns towards him and the bathrobe is nowhere to be seen - a couple of seconds later he starts to put it on again.
Continuity mistake: The patient Zoe is in the ER with her boyfriend while Doug, Carol and Lydia work around her. At one point she puts her hand up to her throat, but in the next shot it's down by her hip, without enough time for the change.
Freak Show - S4-E8
Continuity mistake: Peter, Elizabeth, and Romano are with the little boy who has organ reversal and as Romano says "genetic breakthroughs are in the wings", Peter puts on one glove. As Lizzie asks him if something is wrong, he is putting on the glove again. As they continue to talk, he begins to examine Rodney, but when the shot changes, he is still putting on his gloves.
Of Past Regret and Future Fear - S4-E20
Continuity mistake: After Anna tells Tina Marie she might have kidney problems, Tina grabs her husband Emmett's hand and then puts her other hand over it; the next shot shows her hand is still on the tray and then she moves it to cover Emmett's again.
Other mistake: After resusitation attempts fail, Lucy is left alone with a dead patient whose phone starts ringing. At that point we can see the "dead" patient's chest rising up and down as he breathes (and he's not on a respirator).
Continuity mistake: When Carter comes into the ER with breakfast for everyone, he slips on the muddy floor and the food goes flying. A cardboard tray flies straight up from his left hand but ends up a few feet to his right, which defies the laws of physics.
Continuity mistake: When Mrs. Novotny offers to get aspirin, Angie helps her up by Mrs. N.'s left hand - there's no blood on it, but the next shot shows her hand quite covered in dried blood. Mrs. N. has no blood on her clothes, despite being right next to her husband while he bled to death and Carol administered CPR.
Factual error: Featured characters in the show always seem to be wearing brand new winter clothing - their jackets, hats, gloves, etc. always look pristine, sparkling clean, and just-purchased. Anyone who lives in a winter climate knows that winter clothes age very quickly - the pristine look last about an hour. Extras in the show, probably providing their own clothes, wear much more lived-in normal outdoor clothing.
Ground Zero - S4-E6
Continuity mistake: Carter is talking to Carol about his grandmother. The shot is mainly on her but we can see him put his pen away and pick up a chart - then the shot goes to him and we see him do the same thing again.
Factual error: When the man is brought into the ER after an encounter gone wrong with a dominatrix,Susan checks one eye and announces 'Pupils are equal and reactive' then proceeds to check the other eye. After checking one eye there is no way she would know that the patients pupillary reaction was equal.
Factual error: When the psychotic vet steals an M-60 series tank and drives through Chicago to the hospital, there are several problems with the tank actions. If the vet had combat locked the hatches, the police would not have been able to open the loaders hatch to shoot him, but if it had not been locked, all the bouncing around as he rolled over cars would have bounced it open to lock in the open position. If it had been open and not locked open, it would have been visibly open (8-10 inches) but it was not during all the views on the way to the hospital. During the trip to the hospital, the main gun is in the travel lock over the rear deck, the travel lock cannot be released from inside the vehicle, and if he had ever stopped and gotten out to release it, the police would have shot him, and yet when he gets to the hospital, he can traverse the turret. And while we are talking about the turret traversing, that cannot be done from the drivers compartment, it must be done from either the commander's position or the gunner's position. When the police open the loaders hatch, they shoot straight down into the driver's compartment in the hull, but the vet would have had to be in the right side of the turret, in either the TC or Gunner seat to be traversing the turret.
A Boy Falling Out of the Sky (a.k.a. Shifts Happen) - S9-E15
Plot hole: Dr. Lewis is talking to a patient about pets, and how she is "banned from a pet store" because she isn't good with pets. She explains that the only pet she ever had was sea monkeys as a child. But, in the earlier seasons, she had a cat. We actually saw the cat at one point when her sister was staying with her and let the cat out of the apartment by accident.
Answer: Take care of your dad.
