Revealing mistake: When Mark goes to treat his cut, he leaves the lounge with a couple of tissues held to his head. He gets some supplies and puts a supposedly fresh piece of gauze to his forehead - however the gauze already has some "blood" on it - a feature the camera obviously wasn't supposed to catch. When Jeanie comes over to check it out, he pulls the gauze away from himself and - voila - there's the bloodstain.
Random Acts - S3-E20
Revealing mistake: When Dr. Greene is being attacked in the toilet, he is thrown into a supposedly brick wall. It bends and wobbles like a set wall would.
Revealing mistake: Near the beginning of the show, Mark's father puts a bulb into an old slide projector. Then there is a small explosion with smoke and flames - watched carefully, we can see are actually coming from the other side of the projector - the flames can be seen shooting parallel and close to the floor, which wouldn't have happened if the explosion actually came from the projector.
Revealing mistake: Gallant is watching over some duck eggs that are about to hatch. At the end of the episode, two of the ducklings have hatched and Gallant is showing them to Carter and Susan. An unhatched egg falls out of Gallant's bundle and smashes on the street.The smashed egg has a large yellow yolk and clear fluid, like an egg in a grocery store. But an egg that close to hatching would have almost no yolk remaining and an almost fully developed duckling inside.
Revealing mistake: As Carter walks into the baby's hospital room to see why it is "crashing." In the long shots the baby is obviously a doll, and its mouth is closed even though it is crying.
Revealing mistake: Just before the bowl of pablum falls onto Peter's head, look carefully to see a white adult hand (looks female) holding baby Reese's spoon, and push the bowl so it flips over onto the actor's head.
Revealing mistake: As Dr. Benton is leaving the nursery, there are two people who are supposed to be walking down the hall. But, it is very obvious that they are waiting on their mark, waiting for the go ahead before they start walking. (00:38:40)
Try Carter - S11-E3
Revealing mistake: Ray Barnett is shopping down the cereal aisle and a package of brats is thrown into his hand when you can see there is nothing on the shelves other than cereal. (00:39:01)
Revealing mistake: Neela is delivering a baby. As the camera moves around a nurse to show a close up of Neela and the baby, you see her holding the "head" about a foot away from the mom and quickly moves it closer to look like it's coming out of the woman.
Motherhood - S1-E25
Revealing mistake: When Susan and Chloe's mom comes to the hospital to visit, Chloe gives the baby to Susan to hold. If you look closely you can tell the baby isn't real but is actually a doll.
What We Do - S15-E18
Revealing mistake: Sam's mother is standing at an open window, smoking a cigarette. The outside of the window is covered with some sort of frosted glass or ice or something (making it pointless to have it open anyway). Two shots later, we see her close the window from a different angle. This time, the window is clear and you can see the brick building next door. (00:19:36)
Revealing mistake: After Mark gives his father the keys and tells him to park the van, in a following shot when Elizabeth joins the gurney rush with Carter, as she's walking backwards there's a distinctive large bulge at the back of her waist from the mic transmitter. (00:10:55)
Answer: A man named Paul Sobriki comes into the ER in the season 6 episode 13 " All in the Family." He is complaining of headaches, Carter and Lucy decide to rule out meningitis and do a lumber puncture. Paul struggles during the procedure and believes that both doctors are stabbing him. Lucy, who is interested in Psych thinks something is wrong with their patient mentally, but Carter is dismissive of her. She gets cross with his attitude and she calls the psych department anyway to have a look at him. Later on and near the closing 20 minutes or so of the episode, all the staff start playing loud music and get a cake which appears to be blue lol, together to party as it's Valentines Day, Carter returns from somewhere and asks where Lucy is. He goes to check one of the rooms for her and Paul Sobriki comes up behind him in the dark and stabs him twice in the lower back, he collapses and as he falls unconscious he sees Lucy on the ground also. In the next episode, Be Still My Heart (ep 14), Dr Weaver discovers them and all of the staff work to try and save them, whilst Carter survives. Lucy's wounds are fatal and suffers a pulmonary embolism as Corday and Romano try to save her. Carter is grief stricken and feels guilty about the way he treated her, he becomes addicted to painkillers and Benton, Greene all help him and book him into rehab.
