The Human Factor - S3-E16

Revealing mistake: When Robert is shot, the car blowing up in the alley is sort of half an explosion, abruptly curtailed. Severely cold weather during shooting literally froze the cameras and left only a few seconds of usable footage. That snippet is repeated for effect, but the explosion still looks "cut off" because it is. (00:32:00)

The Dead Zone (1983)

Revealing mistake: The stunt dummy that falls from the balcony in John's stead is pretty obvious. Its blank face is even visible briefly as it falls, and again when it lands. (01:37:40)

The Dead Zone (1983)

Revealing mistake: The outline of the blood pack is visible under John's shirt when Stillson's body guard shoots him. (01:37:35)

The Four-Steps Affair - S1-E21

Revealing mistake: Solo is driving on a country road through a wooded area with no street lights. So what are all those bright round lights reflecting on the hood and windshield of his car? UFOs? (00:10:10)

The Way to Eden - S3-E20

Revealing mistake: Due to close-ups that were cut in backwards, Kirk appears in reverse, with his uniform insignia on the wrong side, twice in this episode: once in the corridor outside sickbay, and again near the shuttle craft on Eden. (00:18:15 - 00:46:55)

The Way to Eden - S3-E20

Revealing mistake: A re-used sickbay scene taken from an earlier episode causes Nurse Chapel to instantly revert to her old hairdo as the sound waves make her fall unconscious. (00:41:00)

Requiem for Methuselah - S3-E19

Revealing mistake: When Spock examines the unknown Brahms waltz, we get a close-up of the music. Unfortunately, the hand-written score bears no resemblance whatsoever to the piece Spock has just played, though he expressly identifies it as such. (00:19:10)

Shore Leave - S1-E16

Revealing mistake: Stock footage turns the single plane strafing Rodriguez and Angela into two attacking planes for a moment. It becomes one plane again when it flies over Kirk and company. (00:33:40)

Black Buddha (1) - S3-E1

Revealing mistake: When Nat enters Nick's loft from the lift, the bright lighting reveals that there is no break in the solid floor to allow the movement of a real elevator. (00:36:45)

Let That Be Your Last Battlefield - S3-E15

Revealing mistake: Stock footage strikes again when Kirk orders the red alert. A shot of the bridge and view screen, with Kirk standing in front of his chair, once more switches Kirk out for a stand in, and Chekhov has gone and is replaced by what looks like from behind like a young woman or a man with a long neck. Also the central display between navigator and helm is now different from that earlier. (00:11:15)

Elaan of Troyius - S3-E13

Revealing mistake: During the battle scene, the use of special effects stock footage of the view screen, as seen over the helm from the opposite side of the bridge, briefly turns the short, brunette Ensign Chekov into a tall, thin blond guy. (00:44:20)
