Continuity mistake: When Megan is running late to pick up her daughter and friends from a sleepover, she arrives in the medical examiner car with Peter. The girls walk towards the car. Megan then gets out and introduces herself to the friends. The girls dump their bags on the ground and the two friends go to get in the car. Her daughter stands by the bags and asks 'who are you?!' to Peter. He explains who he is and that he works with her mum. - however in a previous episode her daughter goes to her work place and interviews all the staff, including Peter. (00:17:35)
Factual error: Bill wears four medal ribbons, in order: Diamond Jubilee Medal (awarded to all people with five years' service in a British uniformed service in 2012), Golden Jubilee Medal (the same in 2002), Queen's Police Medal (a very prestigious award, a handful of which are awarded every year to long-serving police officers for exceptional service) and Police Long Service and Good Conduct Medal (awarded for 20 years' unblemished service in the British police). Not only are these in the wrong order (it should be QPM, Golden Jubilee, Diamond Jubilee, LS&GCM), but he's not entitled to any of them! He's an American who hasn't previously served in the British police. A chief constable's uniform doesn't just come with medal ribbons for the look of it; only those that have been earned may be worn.
Visible crew/equipment: When the truck carrying the caged gorilla pulls into the alleyway, just as Hans and Carl exit the truck and walk away, the blue-sleeved arm of the cameraman/crew member is visible as it moves upward, at the right side of the screen.
Plot hole: Live 50th anniversary episode, aired December 9th 2010 - When Sally is allowed into the rubble to comfort Molly as the firemen attempt to free her, one of them says they'll get Sally a hard hat. When we see her talking to Molly in the rubble, she has no hard hat on and instead one of the firemen is hilariously holding his hand above her head.
Revealing mistake: (Part 4) After Omega takes off his mask and looks in the mirror, when he sees that his physical body doesn't exist he puts his mask back on and screams, but when he shouts, "If I exist only by my will, then my will is to destroy," Omega's mask pops up off his face, and we can see the actor's blackened face as he shouts the last bit of his dialogue. Continuity mistake: Season 4, episode 10 "Ben 10 vs. The Negative 10, Part 2": When the Forever King makes his announcement "Nothing will stop this little home-coming", he stands outside the Mount Rushmore base on bare soil. But in the scene where Max reviews the recording of this speech, all of a sudden we find the Forever King standing on a neatly tiled floor.
