Continuity mistake: When Doug is sitting at the cafeteria with Mark, Peter and baby Reese, his arm is stretched out on the table as Peter says, "fifteen minutes of quality time". The shot changes and suddenly there is no sign of his arm.

John Carter, M.D. - S2-E22

Continuity mistake: Jerry is eating a hot dog while he talks to Carter. He is holding it with one hand. The shot changes and suddenly he has two hands on it but then the next shot goes back to the first position.

Hell and High Water - S2-E7

Continuity mistake: Peter is standing next to the bed of a the little girl who was hit by a car, talking to her parents. Suddenly there is a paper in his hand which hadn't been there in the shots before.

Ambush - S4-E1

Continuity mistake: Mark is intubating the man who is paralyzed from the neck down. We can see his hands working - we know it is him by the splint on his wrist and he is talking as he works. However, Malik comes in and calls him away, and he rushes off, but the hands working on the patient don't change.

Rites of Spring - S5-E19

Continuity mistake: Yoshi and Mark are in the room with ballroom-dancing patient. Yoshi is writing on the chart - when the shot changes the paper on the chart is tented up, with no time for it to change position. When the shot changes again, he has the chart down on the instrument tray, again with no time for the change.

Masquerade - S5-E5

Continuity mistake: Carter suggests Lucy find the drunken patient a detox bed and the pile of books, files and fake arm she is carrying shift position from one shot to the next.

Men Plan, God Laughs - S1-E23

Continuity mistake: Doug introduces his girlfriend's son Jake to old girlfriend Linda who is holding her coat over her arm with her hands clasped together. As she says hello to Jake she puts her hand up to her hair, but from his angle we still see her hands together. This continues for the next couple of shots as they chat.

Of Past Regret and Future Fear - S4-E20

Continuity mistake: As Carter's grandmother is asking him if he'll be at his grandfather's dinner, we see him pick up a mug by its handle - but the next shot, from behind his shoulder, shows him holding the mug by the side.

Make a Wish - S3-E21

Continuity mistake: When Peter goes into Carla's room to talk to her, we see him let go of the door, but in the shot showing him entering the room, he is still holding the door.

The Storm (2) - S5-E15

Continuity mistake: When Kerry and Doug get to the peds unit he starts listening to the kids heart/lungs with a stethoscope. The camera cuts to Kerry and then back to Doug and he is just taking the stethoscope from around his neck. The camera cuts to Kerry and then back to Doug and its around his neck again. The camera cuts to Kerry.Doug's listening with the stethoscope again.

A Bloody Mess - S4-E17

Continuity mistake: Just before Peter bends over beside Reese, an extra is starting to walk behind them. The next shot shows the cereal on Peter's head and we should be able to still see the extra, but there's no sign of him.

My Brother's Keeper - S4-E16

Continuity mistake: At the beginning of the show, Doug is working on the computer while Mark is talking to him. At first there is a document sitting on top of a thick black book but that disappears after a couple of shots - and neither of them touched it.

Good Luck, Ruth Johnson - S5-E9

Continuity mistake: A detective goes into Wilson's room to talk to him, and Carol joins them, sitting near the end of the bed by Wilson's feet. A couple of shots later, as the detective talks to him, Carol is suddenly sitting by his knees, even though she never changes positions.

Benton Backwards - S7-E4

Continuity mistake: The timeline makes no sense. Carter goes home early and is working out in daylight at home. Kerry needs to leave at 6pm and Mark has a hockey game at 8pm. Luka and Abby are out on a date, after dark, when they are mugged, next Mark is at the hockey game. But shortly after, Kerry is getting ready to leave when Carter comes back to finish his shift. After that, Elizabeth is back in the ER working. Nothing in this timeline makes any sense.

Exodus - S4-E15

Continuity mistake: During the last 2 minutes, Jerry is patronizing Dr. Waver. As he walks away, you see Dr. Greene (who has not been in this episode until now) walk across the hall in the background. A few moments later, outside, after Dr. Carter finishes chatting with the fire chief, he bumps into Dr. Greene outside. Dr. Greene is on his way in for his shift starting in 5 minutes and explains that he is coming straight from the airport. (00:43:22 - 00:44:50)

All in the Family - S6-E14

Continuity mistake: This episode is an instant continuation of episode 13. At the beginning of the episode, Kerry is walking in and asks if Mark is still on. Carol says he left at 8 for a double date. However, in the last episode, Mark was already at a restaurant with Elizabeth and their parents when Abby pronounced a patient's T.O.D. As 19:19. The Letter - S8-E20

Continuity mistake: When Dr Greene opens his locker for the last time, the locker is hinged on the left. However when Kerry and Carter clean out Dr Greene's locker after his death, the locker is hinged on the right.

Be Still My Heart - S6-E13

Continuity mistake: When young Robbie stands at the doorway of the trauma room with his father's body, the green door to the back supply cabinet is wide open and the floor is relatively clean, but when Robbie stands beside his father there are changes such as that cabinet door is closed, and the floor is littered with used latex gloves, etc. Then the cabinet door is wide open again when Carol and Luka walk in. Be Still My Heart - S6-E13

Visible crew/equipment: When the son of the accident victims is being led through the trauma rooms to see his parents' bodies, you can see Laura Innes crouching under a table in the background directing the scene. Innes was the director of this episode and it was established that this episode took place on her character's day off. Therefore, she should not have appeared in front of the camera at all.
