Visible crew/equipment: While Tony and Paulie are alone aboard the rented boat, Tony questions Paulie about being the joke leak. When Paulie denies it and says, "It wasn't me, Ton'... That's right," Tony lifts his beer and drinks. It then cuts to a wide shot of the swaying boat, and at the top of the screen, we see a hand move atop the steering wheel at the helm; we even see the top of the hair of this crouching crew member as the wind blows, right before this shot cuts away.

The Case of the Lucky Loser - S2-E2
Visible crew/equipment: After leaving her husband at train station, Harriet takes a cab to meet a man at a cabin. After a few mins, she takes the man's car to go home. As she drives away, there's a reflection of a man, in suit with glasses (possibly the director) standing offscreen. (00:05:20)

Visible crew/equipment: After Sansa tells young Ned Umber to hurry back to Last Hearth and bring his people to Winterfell, when Jon tells Maester Wolkan to send ravens to the Night's Watch, in the next shot of Wolkan the chalk T-mark can be seen on the stone floor in front of his feet (it's not the stone's edges or cracks). Visible crew/equipment: While Jaime talks to Malcolm on the street and takes his picture it's being filmed from two camera angles. When Malcolm shoves Jaime and he falls to the ground we can see the second camera's tripod leg at the right side of the screen, and as Jaime gets up we can see the second camera as well.

The Rex Montana Story - S1-E35
Visible crew/equipment: During Flint's observation of the pow-wow, tire tracks from the camera truck are visible in the background.

Strange Days - S3-E7
Visible crew/equipment: When the guys are talking to Ari via cell phone from their black SUV, if you look at the black area between the front and back windows, you can see the reflection of the camera filming The Scene from outside the vehicle.

Blood Money - S10-E8
Visible crew/equipment: During the court case when Hamilton Stewart is taking the stand and answering to McCoy you can see the white screen in the reflection of his glasses very clearly.

Visible crew/equipment: When Claire runs away from Bob after he calls her "Ms. Bennet", you can see Bob's shadow and another person's shadow on the ground. Since no one else was standing next to them, it has to be a crew member, most likely a camera man based on the odd shape of the shadow.

Visible crew/equipment: When T.J. is playing basketball in the garage, before the helicopter scene, you can see the boom mic fly across the top of the screen for about 2 seconds.

Visible crew/equipment: When Audrey enters her father's office to find him in the middle of his Civil War recreation, a boom mic dips into the shot over their heads. (00:18:40)

Visible crew/equipment: In the episode where Hilary poses for Playboy, Hilary's strap-on mic is visible on the back of her red dress when she, Will, and Carlton first enter the Playboy party. It is gone in subsequent scenes.
