Visible crew/equipment: When Leon is walking down the street, right before Crockett jumps in his car, the camera crew are visible in the shop windows the entire length of the scene, moving along with Leon.

Visible crew/equipment: In the scene where Crockett and Tubbs first meet at the deal, after the cops bust the deal too early, Tubbs steals Crockett's boat to get away. As he's driving the boat away, you see a second person kneeling down in the back of the boat, most likely a crew member. Very obvious.
Visible crew/equipment: When Crockett and Tubbs are being escorted by boat to Bramlette's hideout by his wife Cassie, in a shot from the riverbank you see that Don Johnson and Philip Michael Thomas have been replaced by doubles. Although their backs are to the camera, it's plain to see that they look nothing like the actors who they're replacing. Especially Crockett's.
Visible crew/equipment: At the beginning of the show where three guys take a ride in a convertible and one of them spits at a driver in a truck, pay close attention: the cameraman is visible in the reflection of the convertible, along with two crew members.
Other mistake: When Tubbs and Castillo are talking in the entrance way of the Senator hotel, at the end of their dialogue Philip Michael Thomas makes a very clear gesture with his right hand, which is Don Johnson's cue to finish his phone call in the background and approach the others.
Continuity mistake: When Louis Martinez sees the gunman about to shoot Eddie, he straightens up and yells his name. In the wide shot, however, he is back in his previous position, silent, and then starts to straighten up again. The fact that the scene is being played in slo-mo makes it more noticeable.
Continuity mistake: When Crockett, Tubbs, and Lombard walk out of the house, Lombard is shown taking his hands out of his pockets and then rubs his hands against one another. Then it cuts to Vito and Switek on the roof, and Lombard's hands are now in his pockets. (00:33:05)
Continuity mistake: When Crockett, Tubbs, and Lombard are eating, just as Lombard says people in Italy walk around with garlic bread around their necks, he is holding a fork in his hand and it is pointed forwards. When the shot cuts, the fork is pointed downwards. (00:28:25)
Continuity mistake: When the waitress gives Charlie and Librizzi their milkshakes, she places the tray on the table. But in the next shot, she is holding the tray in her hand. (00:07:15)
Continuity mistake: When Evan falls to the ground after he is shot, there are no visible wounds or bullet holes on his shirt. When he is shown on the ground seconds later, a bullet wound is suddenly visible on his shirt. (00:46:20 - 00:46:43)
Continuity mistake: When Evan hugs Crockett and apologises to him, Crockett puts his left hand around the back of Evan's head. When the shot cuts, his hand is away from Evan's head. Then he puts his hand on the back of Evan's head again. (00:35:55)
Revealing mistake: Crockett and Tubbs walk alongside a department store after speaking with Moreno in an alleyway. While this is happening, the reflections of bystanders standing on the other side of the street can be seen in the reflection of the store, who are watching the scene being filmed. (00:19:05)
Continuity mistake: When Crockett is looking over pages of suspects while trying to compare them to a facial drawing, he lifts one page by an inch. Gina then walks over to him, and the page is suddenly lifted almost halfway. (00:15:37)
Continuity mistake: Just before the punks start harassing the truck driver, the punk sitting in the back seat has nothing in his hand. Then, when the truck is first shown in the following shot, he is drinking from a bottle, which has suddenly appeared in his hand. (00:00:37)
Continuity mistake: When the punks are driving at the start of the episode, the punk sitting in the passenger seat reaches down and picks up a comic book. When the shot cuts, he is suddenly wearing glasses and a bottle has suddenly appeared in his hand. (00:00:30)
Continuity mistake: When Crockett, Vito and Switek are having lunch with the rich lady, in the first shot of the scene, Switek has his left hand hanging over the edge of the table. But in the next shot, his left hand is side on the table and is holding onto a glass. (00:45:37)
Other mistake: When Crockett and Castillo shoot Romano, the gunshots are fatal, but no bullet wounds are shown on his abdomen. (00:45:08)
Continuity mistake: When Trudy parks while in the car with Gina and the two prostitutes, Gina's arms are at her side. Then it cuts to an interior shot, where Gina's right arm is suddenly on the back of the seat. (00:33:45)
Continuity mistake: When Vito and Switek are questioning the two prostitutes, Vito starts drinking from a carton of milk, but when the shot cuts, the milk carton is away from his mouth. (00:31:45)
Continuity mistake: When Castillo shows the team the report from Chicago, he is holding it with his hand positioned along the bottom middle side of the folder. But in the next shot, he is holding the folder with his hand placed on the bottom right corner. (00:26:23)
