Visible crew/equipment: At the end of the episode there is a camera visible in the oval office. After President Bartlet and Toby leave the office an attendant enters to turn off the lamps in the oval office. The scene widens as the lamps are turned off such that the audience can see the entire darkened room. The production equipment, a camera on tripod, and even a crewmember's leg are visible between the penultimate and final lamps being turned off. (00:41:50)
Visible crew/equipment: A crew member can be seen to the left of the frame when Eric walks into Simon and Ruthie's room after listening to their conversation about not being punished for leaving the house, and getting lost while trailing the mailman. (00:36:15)
Burning Down the House - S9-E19
Visible crew/equipment: During the scene where Mimi has moved into the warehouse, Drew makes a comment about the TV having surround sound - as the camera pans over him you can see a crew member in the background adjusting a light.
Firestarter - S4-E1
Visible crew/equipment: When Eli is at McShay's bar he hears the ghost beside him say, "Day after day after day," then when the bartender speaks to Eli, in the wide shots the shadow of the boom mic is visible right above the mirror. After Melinda walks in, when the bartender tells her that Eli scared all his business away, we can see in the mirror's reflection the boom mic dipping into view.
Visible crew/equipment: Episode 1065, year 1995. The shadow of a boom mike against the wall is visible as it rises out of the scene when Barnabas and Julia exit the room.
Visible crew/equipment: When Michael breaks into the car, after he drives off you can see a camera standing in the back seat in the car, and it's pretty clear it's a stunt man driving. (Helps to pause at 18:07.) (00:18:05)
Visible crew/equipment: At the mill, when John tells Buck that Willie Brimmer might be at the Montgomery cabin with Jim-Bob, in the next closeup of John we see a billowy trail of cigarette smoke at the bottom right corner of the screen, but the only people standing there are Elizabeth, John, and Buck, and none are smoking. Presumably, it was someone offscreen smoking too close to the camera (similar to a mistake in 1978's "Halloween"). (00:26:05)
Visible crew/equipment: When Jax and Clay are meeting with Alvarez (when they figure out the niners are branching out), the camera man is visible in Alvarez's sunglasses.
Visible crew/equipment: When Dawn Linder pulls up in her car with her daughter Cassie, several crew members are reflected in the side of the car as Walt and Vic approach it.
Visible crew/equipment: At the end of the episode, when Alicia is listening to the audio recording on her laptop in bed, you can see the hand of a crewmember on the corner of the white cupboard at the left side of the screen. (00:42:45)

Visible crew/equipment: In the scene where House and Allison are walking down the stairs, they round a corner and we see a dolly shot down a short hall with a glass wall in front. During this shot, a camera, cameraman, and dolly are visible in reflections on the glass. (00:08:20)
Visible crew/equipment: The episode where Lisa is running away with Mel. (The episode where Steve dies) There is a scene where Lisa and Mel are panicking at a bus stop, Mel hails a Taxi. When the Taxi stops, you can see the WHOLE crew in the Taxis reflection. Cameraman, soundman, equipment etc.
My Best Friend's Mistake - S1-E3
Visible crew/equipment: In the scene where JD and Elliot are treating a patient and JD says "It's the closest thing I had to a date recently" the microphone pops into screen from above at least two times.
Visible crew/equipment: In the scene where Dana comes back from spending the night with her manager, Alice pushes Jenny and runs and dives in bed. You can see a good shot of a camera man in the mirror.
Visible crew/equipment: In the foot-chase-on-the-beach scene, there are tracks in the sand from the camera crew's truck. They vanish and then reappear when the camera angle changes.
Visible crew/equipment: During McGarrett and Chin's interview with the Chinese bookie, the boom dips into the shot several times at the very top of the screen. (00:20:20)
Visible crew/equipment: Approximately 29 minutes and 10 seconds into this episode, right after Sinclair clasps hands with "The One", Garibaldi and Sinclair are on the ground. In the upper right corner of the shot there is a grip with a red baseball cap and red and white shirt on. He has long hair and spiffy glasses.
Visible crew/equipment: When Clark finds Lex in the Luthor mansion, they walk down the hall and into a new room. As they enter, as Lex is saying "So, hows the new ride?" if you look in the bottom left of the screen you can see a white tape marker on the floor, where Lex will walk over to subsequently. Visible crew/equipment: During one of Tony's coma dreams, when the hotel elevator is out of order, Tony uses the stairway and then ends up falling down a flight of stairs. Just before he lands, we can see a crew member's hands holding a camera steady (filming the stunt double's close-up view) at the bottom left corner of the screen.
