Revealing mistake: In the beginning, during the riot outbreak, there is a scene with a soldier being pulled down from climbing a fence. In the next shot as the crowd is pushing forward, you can see a concrete road block tip over. If you look closely at the bottom of the block, you can see that it's not a real concrete block, but a prop made of wood planks and grey painted rubber.
Revealing mistake: In the scene where Peter saves the child (Venus Lam) from the burning building, right after the jump over the floor opening, the child stands up to help Peter we can see she wears knee pads under her pajamas.

Revealing mistake: When the chair moves across the kitchen floor, Craig T. Nelson goes to investigate it (after it has stopped moving). He goes up to the chair and flips it over. If you look closely, Craig almost hits the camera with the chair's legs. You can tell this has happened because the camera moves up very quickly and Craig looks straight at the camera crew with very big guilty eyes. Revealing mistake: During the big fight scene, just as Annie Wilkes falls to the floor and hits her head on the typewriter, the actress is replaced by a really bad looking Kathy Bates dummy!
Revealing mistake: In the shot where Dick Jones is falling from the top of the OCP building, both of his arms appear to be about a foot longer than they should be. (01:36:30)
Suggested correction: She doesn't put her foot in its mouth. She instinctively tries to kick it or she's just unlucky with where she moves her foot.
