Continuity mistake: Before Ava walks into Henry's office, there are close-ups of the TV, and there are six large books lying beside the TV. The top three books do not have their binding facing out, but when Ava walks in, all the books have their binding facing out except the top one.

Continuity mistake: When Ava goes to Henry's office to bring him the files, he stands in front of the TV after she makes a crack about it. In the shot facing him, he's holding the files with his right hand and turning off the TV with his left hand, but in the next side shot, it's vice versa.

Continuity mistake: When Johnny and Roy are heading to the parade in The Old Engine, and get sidetracked dealing with a fire rescue, they are wearing the old-style shirts with a ten-button, square bottom bib front, with five buttons on each side. When Captain Stanley laughs at them on the roof, their shirt style differs in the close-up, it's now a nine-button, pointy bottom bib, with four buttons on each side and one button at the bottom point.

Continuity mistake: When Johnny and Roy are treating the officer, while hiding between the bakery truck and car, the sniper fires at them which leaves multiple bullet holes on the side of the truck. However, when Vince talks to the Sergeant on the handie-talkie and then fires the tear gas, all the bullet holes on the truck are gone, but they soon reappear.

Continuity mistake: When Johnny goes to the closet to get the mop, because he's 'latrine officer of the day', he gets hit by the phantom, and all the guys run out of the kitchen to see Johnny wet again, so when dispatch drops the tones they all head for the vehicles and Captain Stanley removes his jacket. Then next shot all the guys are running out of the kitchen, as if they were all still there, and Stanley is wearing his jacket.

Continuity mistake: After the Doctor yells at Handles for putting him on the Dalek ship, the Doctor grabs him and carries the head with his right arm, then switches the head to his left arm. However, in the close-up as the Doctor asks, "What does this question mean?" we see Handles is back on the Doctor's right arm, but next shot as the phone rings, Handles is carried on the Doctor's left arm again. Continuity mistake: When Roy walks into the kitchen at Station 51, Billy is on the couch studying, and in the overhead shot the pages of the open book that Billy is holding has illustrations, but in the close-up the pages have only text on them. Plus when Roy is in the doorway, you can see the truck (s) behind him, but when he turns to go back to bed, the garage is empty.

Continuity mistake: When DeSoto and Gage are in the pool with the little girl whose arm is stuck in the drain, their shoes and pants are soaked. However, just after Gage gets off the Biophone, his shoes and pants are completely dry, and then when the pool has been mostly drained both DeSoto and Gage are dry.

Continuity mistake: When Gage sees Sheila in the hallway at Rampart while she's undercover, he stops her midway between treatment room three and four and they are standing face to face. In the wide shots of both of them and in Sheila's close-ups, she's standing right in front of the emergency fire hose glass cabinet, but in Gage's close-ups, he's standing directly in front of the door to treatment room four, farther up the hallway.
