A New Evil: Part 1 - S4-E1

Revealing mistake: After Joey says "Let's pay a visit to Kaiba" and Yugi replies "Good idea", the scene cuts to a grey car slowing down while a purple dragon lands on the ground in front of it. When this happens, there are two background extras to the left of the shot who are standing completely still and motionless. (00:07:59)

The Creator Returns - S4-E4

Revealing mistake: After Tea points out that Pegasus doesn't have the Millennium Eye anymore, there is a wide shot where Tristan says, "Good point, last time we saw him, he was a little less creepy." In the shot, the card that came in Pegasus' package can be seen on the table and is missing the illustrations. (00:04:57)

Revealing mistake: After the briefcase has been removed from Kaiba's jet, there is a shot of Kaiba and Mokuba standing outside the door. In the shot, the KaibaCorp logo on Kaiba's belt has been drawn wrong, where there is usually a "KC" logo on the belt, but there is a "KO" logo instead. (00:02:21)

The Challenge - S4-E11

Revealing mistake: While driving in Devlin's car, the image of the map in Tea's lap can be seen shifting unnaturally back and forth as the car drives. This was likely done to make it appear as if the map was vibrating due to the movements of the car, but the outline of the map remains completely still, making for a completely unnatural animation. The Fate of the Pharoah: Part 1 - S4-E12

Revealing mistake: While talking with Rafael shortly before the duel, there is a shot of Rafael saying, "Look, the name's Rafael and I called you here to duel," where Yugi is standing in the foreground of the shot. Revealing mistake: When Kaiba's men are typing at the computer while the thugs are attacking, there is a shot of a man typing at a keyboard. A Duel with Dartz: Part 1 - S4-E33

Revealing mistake: When everyone is on the helicopter after the building collapses, they realise a temple has emerged in its place. In the shot of Tristan saying, "I guess that's what Paradiaz Headquarters really looks like," the Millennium Puzzle that Yugi is wearing is notably smaller compared to every other time it is shown. A Duel with Dartz: Part 1 - S4-E33

Revealing mistake: When the captain is explaining that there is nothing they can do to stop Dartz, there is an exterior shot shown from above of the three ships in the ocean. The helicopter rotors on the middle ship are shown moving, indicating that this is not a still frame; the ships and the water turbulence caused by the ships remain completely still in the frame. A Duel with Dartz: Part 3 - S4-E35

Revealing mistake: After the Joey knight has blocked Kaiba's attack on the Mai knight, Kaiba says, "Figures Wheeler mess things up," and then inserts a card into one of the slots on his duel disk. The slots on the duel disks have always been shown to be aligned with monster card zones on the front of the duel disk, but when Kaiba sets the card, he inserts it in an area in between where the monster card zones are located, meaning he inserted the card in between the slots, rather than in one of the slots. A Duel with Dartz: Part 3 - S4-E35

Revealing mistake: When Rafael confronts Dartz after arriving at the castle, in the shot of him saying "Don't answer that, I've heard enough lies from you, the Pharaoh opened me up to the truth," Yugi can be seen in the background of the shot, but his Millennium Puzzle is missing the designs that are usually shown on the front. The Tomb-Keeper's Secret - S2-E46

Revealing mistake: Whilst discussing Marik with Ishizu, there is a wide shot of the bedroom where Joey says, "What if it's too late to save Marik from the evil that's inside of him?" In this shot, there is an animation error with Serenity's head. Serenity has her head turned to face Ishizu, but the turned head appears to have been poorly pasted over another version of Serenity's head that is facing directly forwards. (00:04:44)
