The Changeling - S2-E3

Visible crew/equipment: When Kirk walks into the brig Spock tells him that Nomad has almost human stubbornness, and that its screens are still up, then just as Kirk moves closer to Nomad the shadow of the moving boom mic is visible at the top, left side of the screen. (00:28:15)

Super Grover

Miri - S1-E9

Visible crew/equipment: As Spock and the guards edge toward the mouth of the alley, watch the upper part of the wall to their right. There are shadows of somebody's hands throwing the debris down, carefully avoiding direct hits.

Movie Nut

Mudd's Women - S1-E7

Visible crew/equipment: In the briefing room, as the camera moves to its left along the seated figures, the camera rig is reflected in the glass of the communication view screen.

Tomorrow is Yesterday - S1-E20

Factual error: Towards the end of the show the Enterprise is leaving Earth orbit and heading towards the sun. We see the Earth diminish and the moon appear looking exactly as it does from Earth. From this angle we should be seeing the "dark side" of the moon, which looks completely different. (00:40:50)
