Continuity mistake: When Bart leaves the school, you can see that his skateboard is green and perfectly flat. But when he is skating through the crowd and past the people at the bus stop, the skateboard has an orange stripe down the middle and is raised at the rear.
Suggested correction: Since there's time between when he leaves school and skates past the crowd, he could have grabbed another board.
Tree House of Horror XII - S13-E1
Audio problem: In 'Wiz Kids,' when Edna asks Harry Potter if he is chewing gum, he replies "No ma'am, it's brimstone," however it's clear that a different line was originally intended. (00:15:25)
Suggested correction: I just rewatched this scene and his mouth visuals match with what he's saying; no mistake here.
Homer's Barbershop Quartet - S5-E1
Continuity mistake: In the flashback where Marge is making a model of Homer, the models head falls off and Santa's Little Helper picks it up, but the Simpsons did not own the dog back then. (00:17:40)
Suggested correction: How do we know it's not another dog? Though not likely, they could have had a previous dog that highly resembled Santa's Little Helper.
And Maggie Makes Three - S6-E13
Continuity mistake: When Homer remembers finding out that Marge was pregnant with Bart, he screams, tears out his hair and runs upstairs, past a picture of Lisa in the pink hat she wears to church. This mistake re-occurs seconds later when Homer remembers finding out Marge was pregnant with Lisa. Hard to have photos of Lisa on the wall before Bart and Lisa were even born...
Suggested correction: The picture isn't clear enough to show that it's Lisa exactly. Also, according to the overall Simpson family tree, one of Lisa's relatives had a very similar hairstyle, and it could have even been another one that decided to change their hairstyle. It is normal to have pictures of cousins and previous/current relatives around the house.
Continuity mistake: When Bart's army is attacking Nelson, there are a lot more kids attacking Nelson than there were training with Bart.
Suggested correction: 1) Bart could have recruited extra help last minute. 2) There was time between the military training montage and when they attacked Nelson and his friends. Bart could have trained them as well.
Audio problem: At the theatre, when the cook talks to Mark Hamill off-stage he sounds exactly like Moe the bartender.
Suggested correction: 1) Though very similar, there is a slight difference between the cook and Moe's voice if you listen carefully. 2) I have a lot of friends who sound like each other, some even exactly the same, so this wouldn't be a mistake either way.
Audio problem: When Bart calls Milhouse to get the answers to the history test from him, Milhouses voice sounds a lot different from his usual voice. It sounds more like Mrs. Krapabbel's voice. (00:08:30)
Suggested correction: His voice sounds exactly as it should; no mistake here.
Continuity mistake: At the bar, we see Mrs. Krabappel, and she disappears for the rest of the scene. (00:13:40)

Continuity mistake: When Bart crashes at the door of the Legitimate Businessman's Social Club, his head is facing the door. You can only see this for several frames. In the next shot, his head is facing the sky. (00:05:25)
Tree House of Horror VIII - S9-E4
Other mistake: In 'Homega man', Homer goes into the bomb shelter and does not shut the door, and you can still see it open for a while after he is in there. Herman does not close it behind him - you can tell by his skeleton's position. But Homer opens the door to leave the shelter after he has eaten the food, and it must have been closed for him to have survived the atomic attack, because it vaporised Herman, who was only a few feet away.
Suggested correction: There is plenty of time in between when Homer enters the bomb shelter to when the neutron bomb hits Springfield. Homer or Herman could have closed it then.
Continuity mistake: At the beginning, when the bus is driving to the school, the streetlights outside disappear/reappear between shots. (00:01:30 - 00:02:20)
Audio problem: Where Lurleen sings the line "I tried to kiss him," on the TV show Ya-Hoo!, you can see that her lips aren't moving. (00:20:35)
Continuity mistake: While Homer is sleeping, his head is facing the 'Core temperature' indicater. When he wakes up, he is in a different position. (00:04:25)
Continuity mistake: When Bart finds out that Homer took the police badge out of the cereal box, it changes from brown to pink. The food inside the box also changes to pink. (00:01:10)
Missionary: Impossible - S11-E15
Continuity mistake: This mistake happens twice in the episode. At first, when Homer arrives on the island, he gives the islanders some bibles and says "Knock yourself out." Ack says that they cannot read. Yet later when Homer is revealing the casino, Ack reads "The Lucky Savage-?"
Suggested correction: Homer had been there for a while when the casino had finished construction. He could have taught Ack to read.
Burns Verkaufen der Kraftwerk - S3-E11
Continuity mistake: When Carl complains that he can't be fired, everybody behind him, Homer and Lenny has disappeared. (00:09:00)
Other mistake: When the wheel falls off Bart's skateboard, in the shot where he curves left sharply, you don't see the school in the background. How could he be at the social club if he just left school? (00:05:20)
