Continuity mistake: Though he's leaped into someone else, we always see Sam as Sam, dressed in all the host's clothes (including glasses or sunglasses). His host is revealed only in reflections. But here, when he looks at his host-self in the mirror, the hat and clothes are all identical - except that the reflection is wearing glasses, and Sam isn't. (00:43:00)
Tiger Left, Tiger Right - S2-E6
Continuity mistake: Kimble's been kidnapped and held for ransom. He had nothing with him when he was taken, yet when he leaves the abandoned house with the kidnappers, he has a small suitcase. When they drop him off on the road, he doesn't have it. When, in the epilogue, he's shown walking, he's carrying a different, larger suitcase. (00:46:00 - 00:49:00)
That Night, A Forest Grew - S2-E7
Continuity mistake: When Deb meets Lila, Lila is at the fridge and puts her arm over her breast - she isn't wearing the 'engagement' ring. However when she returns to Dexter's room it's on her finger.
The System - S3-E15
Continuity mistake: Phelps takes the tape from the recorder's right-hand spindle. When he opens the acid vat, we can see the tape he just removed back on the recorder - but on the left spindle. The tape he then drops into the acid doesn't remotely resemble the one he pulled, and when he drops it, the recorder's left spindle, where the full tape was in the last shot and where the empty reel should be, now has nothing on it at all. Continuity mistake: When Pete is trying to get the drill back from Ben (before Pete says "I'll give you a fiver if you give me the drill"), the position of Ben's hands on the drill change between shots.
Continuity mistake: Rex's kama (the black kilt-like thing he wears) is missing twice: first when he, Anakin and Ahsoka are looking at a hologram of an AT-TE on the Resolute's bridge, and second when Anakin's trap for Grievous is revealed, and Rex orders the AT-TEs on the asteroids to fire. Continuity mistake: When Serena is saying goodbye to Dan, rain is pouring on the angle focusing him, but not on the angle focusing her. This changes back and forth.
On The Edge - S2-E9
Continuity mistake: When Al goes to the ship to get the explosive, his dingy is powered by a Seagull outboard. When he returns, its a Mariner.
Charles (Manson) in Charge - S7-E10
Continuity mistake: Kai applies shaving soap to his whole face, including his upper lip. In the next shot, there is no soap on his upper lip and he puts it on again.
Continuity mistake: When the doctor is putting the bandage on Laura's hand, it ends at her wrist. But later, the bandage goes all the way to her elbow.
Continuity mistake: In the final scene, John Ross decides to drink some of the whiskey from the bottle that his father, J.R. Ewing, left for him. The bottle of whiskey is about half full, filled to just about the bottom of J.R. Ewing's signature on the bottle. As John Ross reaches for the bottle, it is now suddenly only about 1/6 full compared to the half full it was from the previous shot. The level continues to move up and down. (00:41:45)

Continuity mistake: When the wine falls on Toby his shirt is soaked and turns pink. The camera slam cuts to Happy, then back to Toby, whose sleeves are now white with red splotches instead of pink. (00:29:45)
Continuity mistake: A criminal steals Fish's gun and holds the squad hostage. He has a handcuff on one wrist with the other one hanging. The hanging cuff goes back and forth between being open and being closed throughout the scene.
Continuity mistake: Elizabeth's hand on Gabriel's arm changes positions shot to shot.
Continuity mistake: When Riggs is talking to Agent Palmer about being hungry, he's holding one red folder. When he asks if she was going to eat, he sets the folder down. In the next shot, he's holding a folder again.
Always Waiting for the Next Bad News - S3-E9
Continuity mistake: Jessica is wearing a massive earring, but when the camera is behind her, she isn't. (00:23:38)
Continuity mistake: When Midge is doing her makeup routine, she doesn't have eye liner, then she does, then she doesn't.
Continuity mistake: When James is taking pictures from a car as men are entering a hotel, in the shots of him head on he is using a Canon EOS camera with a 70-200mm white lens. In shots from the side he is using a Nikon camera with an older Nikon lens. (00:27:35)
Kids Today - S3-E1
Continuity mistake: When the father arrives at his son's car crash, there is a low shot looking up, specifically at the father. The sun is almost overhead. Cut to the rest of the action, the sun is further down in the sky.
Continuity mistake: When the Polaroid photo is shown of Betty and Walter with his boss and his wife, Betty's hands are behind Walter and the boss's wife's back. When it was taken, her left arm was out and her hand was on her own leg.
