Continuity mistake: When Megan is running late to pick up her daughter and friends from a sleepover, she arrives in the medical examiner car with Peter. The girls walk towards the car. Megan then gets out and introduces herself to the friends. The girls dump their bags on the ground and the two friends go to get in the car. Her daughter stands by the bags and asks 'who are you?!' to Peter. He explains who he is and that he works with her mum. - however in a previous episode her daughter goes to her work place and interviews all the staff, including Peter. (00:17:35)
Continuity mistake: Ruth is seen in a flashback. The young Ruth has a mole on the left side of her face, as does the adult Ruth. But in one shot, when she is slapped by her father, the mole appears on the right side. (00:20:00)

Continuity mistake: When Steve gives Fiona the ticket to Miami he says it is out of O'Hare. When he goes to the airport he is at Midway.

Continuity mistake: When Fidel and Dwayne arrive at the place where Camille and Richard are hiding for the hurricane, they can open the doors without a problem, whereas Camille and Richard had taken great care of locking the doors from the inside.
Continuity mistake: When Hook comes to see Emma at the sheriff's station, his earring disappears when she kisses him goodbye.
Continuity mistake: Carrie Matheson leaves her sister's house in a Jeep Compass with her daughter. When she arrives at her destination, she is in a newer model of the Jeep Compass (the front indicators are different).
The Last Of The Starks - S8-E4
Continuity mistake: In this episode, Kings Landing is shown as sitting on flat, plain-like terrain. In previous seasons, it was shown surrounded by water and mountains covered lush greenery.
Suggested correction: That is correct, however there is actually a reason for this, it was just never adapted from the books. Stains burned the woods during battle of the black water as an offering to the lord of light.
That doesn't remove the hills though.
Lady Killer - S3-E3
Continuity mistake: Root shoots the hitman, walks up to him and slides his gun a couple of feet away. A wide shot shows the gun where it should be. After she says "you're the boss" and starts walking away, we see exactly the same angle, the gun has vanished, and some random black bits have appeared.
Charles (Manson) in Charge - S7-E10
Continuity mistake: Kai applies shaving soap to his whole face, including his upper lip. In the next shot, there is no soap on his upper lip and he puts it on again.
Continuity mistake: Leo speaks to Timo as if it's his first time meeting him, but they've met several times throughout episodes 1 through 9. (00:25:01)
Continuity mistake: When the couple is in the boat when the creature appears, the woman is taking a picture and holding her phone in her left hand, she turns around and her left hand is raised and her right hand is on the boat edge. When the camera angle changes, her left hand is now on the boat and her right hand is lowered and holding the phone (confirmed when the camera angle changes again and she's in the same position, holding the phone in her right hand).
Continuity mistake: Prior to the dinner at Fancy's with George's parents they show the restaurant from outside but the person in the window is the Veterinarian that Zoe stands up later this season and it is actually the same exact shot from when he is waiting for her to show up for their date.
San Junipero - S3-E4
Continuity mistake: Near the end, Yorkie gets into a red left-side drive convertible, then is seen driving on highway in the same car but with right side drive. Following scenes are of a left-side drive car. (00:58:00)
Continuity mistake: When Jack is pouring Amanda a drink at the end of the episode he has a bottle in each hand. In the next shot he has only one, with his other hand holding the shaker.
It's Gonna Kill Me, But I'll Do It - S1-E4
Continuity mistake: The recording of Bridget on the boat that Agent Machado plays is slightly different each time we hear it. The first time it says "Hello? Hello? It's my sister, can anyone hear me?" The second time, she only says "Hello?" once.
Continuity mistake: Early in the show, Poppy Montgomery's character gets into bed with no bra. Yet in her flashback of that night, after rushing from her bed in the middle of the night to the murder victim, a black bra strap is visible. (00:03:15 - 00:04:20)
Continuity mistake: When the detectives are in pursuit in the alley, they are followed by a police car. In all shots from the front it has its blue lights on, while they are off in all shots from the rear.
Continuity mistake: In the scene where Cricket is cutting out clips of the Bill Vaughn scandal, she places the one clipping into the binder on top of another, then in another shot, the new clipping is no longer there, and she places that exact same clipping into the binder again.

Continuity mistake: When Amy picks up her book from the trash her thumb is on the front cover. When the camera cuts and she hands the book to her co-worker, her thumb is on the back cover.
