Revealing mistake: Sloth is shown with three "arms." It's most easily noticed after he wakes up. His right arm is bent at the elbow, pointing to the ceiling, his left arm is pressed tightly to his left side between his torso and the jacket used to conceal the upper part of the prosthesis, and his third "arm", the prosthesis, is bent at the elbow, hanging halfway off the bed, and pointing to the left of his body. It has several large, protruding "veins" in it and the stump where the hand should be is off-screen.
Revealing mistake: In the scene where Lorraine Bracco points a revolver at Ray Liotta's head, after Ray Liotta turns the tables and now points the gun at Lorraine Bracco as he gets up and walks away you can see Ray Liotta steps on Lorraine Bracco's hand and she opens her mouth wide like shes screaming but it must have been cut from the audio, as Ray walks off camera his expression changes to show that he knew he had accidentally stepped on his co-workers hand.
Revealing mistake: When the body of Miss Lefferts is being viewed, the 'dead' girl blinks slightly. (00:34:35)
Suggested correction: It's not uncommon for dead bodies to continue moving after death. Many internal parts of the body continue functioning in some fashion for hours after death and nerves and muscles can twitch.
Revealing mistake: When Sean Penn is in the funeral home looking at his daughter's body, when he lays the dress over her the rise and fall of the very alive actress' chest is visible as she breathes. (01:07:40)

Revealing mistake: On the transport trailer, Frank leans out his car window and dramatically fires at the ramp's tall lever control in a close-up, located at the rear of the transport over the tires. The problem is that the trailer has no such lever control, as seen in the previous shot before it drives under the bridge overpass. (00:07:55)
Revealing mistake: When Tim Curry runs out of Kevin's hotel room after being scared by the inflatable clown doll he hits his knee on a table. When he hits the table and then hops away you can clearly see that he is wearing a knee-pad.
Revealing mistake: In the scene where Norton's wife is target shooting with the revolver, there aren't any bullet heads showing in the cylinder. There's also no recoil as she fires.
Revealing mistake: When three of the turtles are playing trivial pursuit and the fourth runs in to announce Splinter is alive, you can see a wire coming out of Raphael's shell. I guess it's for the animatronic head but it's clearly visible. (00:59:30)
