Revealing mistake: After the first race, when Tej walks over to Brian with a bunch of people, there is a girl in a orange and black top, and as she walks towards Brian, she is looking directly at the place where the camera is supposed to be and backs away, so the camera can pull in for the shot. Revealing mistake: Any time that Charlie is kicked by the kangaroo, they show him falling back onto the ground. But, if you look closely, they just used the same shot over and over. The exact same two lines in the dirt are to his left by his head each time.
Revealing mistake: When the police car crashes through the grating that was dropped down in front of it at the end of the tunnel, the occupants are dummies - they just bounce about in the same upright position whilst the car drives into the river.

Revealing mistake: When the female terrorist goes to use the large drillhammer on the concrete wall, the force of turning it on causes it to "jump" in her hands. When the drill bit bounces against the concrete wall, you can clearly see the wall move back and forward about half an inch.
Revealing mistake: Near the end of the scene, when Bruce and Fox are in the tumbler Fox says "We never could get the damn bridge to work, but this baby works just fine" then Bruce turns the wheel to the left but the tumbler turns right. (00:57:45)
Revealing mistake: After they kidnap Neil, they are driving to stop the wedding, as they are driving down the road, you can see out the front window that the truck pulls up to a stop light. They cut to Neil and the truck is still moving, then they cut back to the group and they road is clear. Traffic lights don't cycle that fast. (01:20:30)
Revealing mistake: When the Senator is being killed by Travolta in the river, we can see some shots of his assistant before his car explodes. If you watch closely just half a second before the frontal shot of the explosion, you can see in the driver's seat a dummy with a green face.
Revealing mistake: When McClane goes into the icy room in the power station, look at the pipe to the side of the acrobat: it's in shot a few seconds and you can see that the writing on the pipe is reversed, revealing that in the editing room, the shot was flipped.

Revealing mistake: When Two-bit Mathews returns from the hospital gift shop with a copy of "Gone With The Wind", he hands the book to the nurse asking her to make sure that Johnny gets it. The book he hands her is an obviously used paperback with the upper front top right corner and pages all bent back. Shouldn't a gift shop sell NEW items? The book is only shown from the non-title side so they could have used any paperback for the prop as long as it looked like it wasn't several decades old.
Revealing mistake: When the Grand Ville turns onto the street where the children are playing, it clips a parked car as it makes the turn. This is the same car that later gets its driver's door ripped off by the Grand Ville just before the police give chase. It was repainted red for the second scene, but note the same dent in the right fender as before.
Revealing mistake: When Hightower is in the Monster Truck you can see that a white stuntman is actually driving it in one of the shots.
