Revealing mistake: During the fight scene in the belly dancer's room, Bond spins the bald guy around to avoid being hit by a chair. When the bald guy gets hit in the back with the chair, you can see a large rectangle of padding under his jacket.
Revealing mistake: After the final fight scene, where Maximus is dead and on his back in the sand, his armour would have made his head droop horribly backwards. To avoid that, the filming crew put a pillow of sand under his head to raise it about three inches. (02:20:10)
Revealing mistake: After Evolet got hit by the arrow and presumably dies, D'leh is staring at a mammoth. Behind the mammoth, you can see that the background is a picture.

Revealing mistake: When the Black Pearl swings alongside the Interceptor for the battle, Elizabeth shouts, "Fire all!" The next shot is from behind the lit cannon which is aimed at the Pearl and the following shot faces the Interceptor's cannon, as it goes off. In the shot facing the Interceptor, the raw wood scaffolding type beams are perfectly visible at the entire lower part of the screen, where the painted planks (below the gun ports, above the bilges) of the hull should be. Revealing mistake: Bond escapes from the buried pipeline when two technicians open a hatch from the outside. One tech unlocks the hatch by rotating a large wheel, which drives the locking mechanism. But when he finally opens the hatch, there's absolutely no mechanism attached. There's just a bare axle that the wheel turns on.
Revealing mistake: During the scene at the restaurant in Montenegro (actually filmed in the Czech Republic), you see a payphone with a Czech Telecom logo on it (itself a piece of history, as these are all now rebranded O2). (01:57:50)

Revealing mistake: In the visions of Nefertiri scene where Nefertiri is fighting Anck-su-Namun back in ancient Egypt, Evie (Nefertiri) gets knocked to the floor and when she lifts her mask up you can see on her right index finger is a band aid/plaster. Revealing mistake: In the scene where Bond is trapped in Largo's shark-infested pool, he uses his mini air tank to breathe. In most of the subsequent shots, look closely at the actor's face - it looks a lot more like a stuntman than Sean Connery. (01:20:20)
Revealing mistake: When the huge alien ship settles over the White House there is an exterior wideshot of the grounds. At the bottom of this shot it's funny how there are many tourists - adults and children, just milling around in front of the gate and across the street with a van driving along between the two sides, and not one has any sort of reaction to the alien ship. This real shot of the White House (some shots had smaller-scale models) had the CG ship edited in during post production. (Visible on fullscreen DVD.)
Revealing mistake: When Inigo and Westley are duelling, they are making athletic jumps. One does it after the other and when they land you can see the mat wrinkle under the "dirt" on the ground.
